Looking for alternative for AML treatment with kidney failure.

Posted by hoppet @hoppet, Jun 13 6:33am

69 year old with kidney failure now diagnosed with AML and already has stage 4 cervical cancer. Dialysis is being tried but we are not hopeful.
Recommendations is palliative care as patient is very weak.
Doctors are supportive but I’m hoping there are other alternative you could recommend.
Thanks

Hello @hoppet. This is such a heartbreaking story. Your dear friend has been facing an incredible challenge for some time with her stage 4 cervical cancer and kidney failure. Now, an added diagnosis of AML is certainly further complicating her condition.
There are different forms of AML, (acute myeloid leukemia); Some being more aggressive than others, requiring different treatments.
To my knowledge these are the possible treatments;
Chemotherapy, Radiation therapy, Chemotherapy with stem cell transplant, and targeted therapy…all of which would be arduous for someone who is already so compromised health-wise, with very limited treatment options.

For now, until her doctors feel she may be able to proceed with treatment for AML, palliative care would help keep your friend (loved one) comfortable while she continues with her cervical cancer care and dialysis for kidney failure.

Palliative care is not the same as hospice. Palliative care is a specialized form of care that provides symptom relief, comfort and support to people living with serious or chronic illnesses. It also provides support to caregivers and those impacted by a loved one’s condition.

Have her doctors talked about any possibility of low-intensity treatments for her AML?

Lori, thank you so much for your supportive and quick response.
Doctors are cautious about starting any form of chemotherapy because they feel the kidney is very compromised so want to try dialysis to look for improvement before any more chemo. Platelets have been low as well so infusion have been given, there is some improvement. The kidney are the focus at the moment.
We have been warned about going too aggressive with treatments so very scary.

Having had AML myself, I think this is a good compromise by the doctors. This is a risk/reward situation right now as far as starting chemo for AML with the kidney impairment.
Dialysis may help with improving the kidney health enough to be able to start the AML treatment in the future but for now, the kidneys need the attention for sure.

Are you the patient or is it a loved one or friend? Has treatment for stage 4 cervical cancer been completed?

Buona giornata

Hi Lori, this is for my mother. Dialysis helped the kidneys a bit. Platelets are low so waiting for review for AML test and options if chemo or immunotherapy can be commenced.

Glad to hear the dialysis is helping your mom’s kidneys improve. That much is a relief. Are you waiting for results of a bone marrow biopsy? Or were these cytology tests to check for any potential mutations for AML? That can dictate the type of treatment.

Hi Lori, additional tests are indicating AML is not present so good news but confusing, was diagnosis too soon.
So since cervical cancer was confirmed, chemo has been ruled out for now due to the renal issue.
Platelets continue to go down.
She’s stable but very weak.

With your mom’s cervical cancer, perhaps blood numbers were skewed with a higher white blood count, or who knows. But take the “no longer having an AML diagnosis”, along with her cervical cancer as a win! The poor dear doesn’t need a double whammy. I’d like to follow along with you and your mom though so please keep me updated, if you don’t mind.

But since her cervical cancer is out of my area of experience, I’m going to post a link for you to our Gynecologic Cancers discussions:
https://connect.mayoclinic.org/group/gynecologic-cancer/
I think this would be a very helpful group for you to get information for your mom. Feel free to look through current conversations and pop in with your questions or start a new one for your mom!
I wish her will with her kidneys and hope they improve significantly so your mom is able to get treatment for her cervical cancer. Did the doctors have alternatives besides the chemo?

Hi @bettersleep68 Good to hear from you! It looks like slowly but surely your blast count is coming down with the Vidaza treatments! That’s positive news but not yet remission. It can take time to get there so don’t get discouraged. How are you doing with the treatments. You feeling ok? What does your doctor say about the results of the treatments.
Was the 40% down to 30% discovered with your most recent bone marrow biopsy?
