What tests are there that will definitely diagnose whether I have MAC/
For years I was diagnosed with asthma. I was hospitalized several times for an asthma exasperation. One time was 3 weeks. Cough, fatigue always got worse regardless of treatment plan. Nebulizer, prednisone. Now second opinion diagnosed me 2 years ago with bronchiectasis. I only take daily trelegy 200 mcg. Just got results to ct scan that I should be tested for MAC. The thing is when I get sick its very sick and last weeks. However, the rest of the time I feel ok. Its worrisome to day the least.

@maureenmc A MAC diagnosis considers a positive sputum culture which should also specify the bacteria, a CT scan and your symptoms.
@maureenmc There are two ways to positively identify Mycobacteria Avium Complex (MAC) also known as Non-tubercular Mycobacteria (NTM) - either 2 or more consecutive sputum cultures or a bronchoscopy.
As of late 2025, there is a new tool in your doctor's arsenal - there is a medication called Brinsupri that may reduce (but maybe not eliminate) the number of infections or exacerbations you have.
If either is positive for the bacteria then a CT scan can be performed, and with that information, a physical exam, and a review of your symptoms, a treatment plan can be formulated.
Treatment plan? It could be the 3-antibiotic-cocktail we call the "Big 3" -either 3 days/week for most people, or 7 days/week for those whose lungs show extensive infection like cavities. OR it could be "watch and wait" - daily airway clearance, with or without 7% saline nebs, with frequent monitoring.
Are you seeing a pulmonologist? Have they suggested a sputum culture to see what recurring infection you are dealing with?
