UTI while on Meds.

Just completed 2 weeks of Bactrim and vancomycin after another UTI.
After a clean culture, taken by the infectious disease Doctor; I am currently on a maintenance dose of both.
Meanwhile I go to the Urologist yesterday. He takes a sample urine, clean as crystal; all good! Today while still on meds, the infection is back. I'm confused and totally at wits end.
