Been on TRT for over 10yrs: Upcoming biopsy & PSA levels

Ive been on TRT for over 10yrs, but was getting things from bodybuilders at the gym, but wanted to start doing it correctly, for im known to have low testosterone. Now that i get my blood work done every 3 months to get my prescription filled, my doctor started getting concerned about my psa levels. I will list below, but in the meantime, ive already seen an urologist, and he said my exam felt normal, but now scheduled me for a biopsy next week. Im surprised they didnt do another blood panel, unless they just went by by records with family doctor. Curious to what ppl think of my scores, and should i be concerned.
10/14/22 = 2.12
6/13/24 = 3.63
6/27/24 = 2.81
12/2/24 = 2.29
5/19/25 = 4.3
8/18/25 = 4.69

Well my follow up with urologist isnt until tomorrow, but i got my MRI back on my portal. Looks like ill be getting that biopsy after all. Here is what it says, but will know more tomorrow after talking to the doctor.

Prostate size and volume: 4.0 x 3.4 x 3.6 cm and 25 cc
PSA density: 0.17 ng/mL/cc
Length of membranous urethra: 15 mm
Hemorrhage: Absent

PROSTATE ZONES:
Transition zone: Nodular BPH changes.
Peripheral zone: Multifocal prostatitis.
Lesion # 1:
- Key image: Series 6, image 20;
- Size: 10 mm;
- Location: left; apex; posterolateral peripheral zone;
- T2WI: 3; DWI: 5; DCE: positive;
- PI-RADS v2.1 score: 4 - High (clinically significant cancer is likely to be present);
- Likelihood of extraprostatic extension: 2 - Unlikely;
- Likelihood of seminal vesicle invasion: 1 - Highly unlikely;
ADDITIONAL FINDINGS:
Urinary bladder: Increased trabeculation and/or wall thickening.
Lymph nodes: Within normal limits
Musculoskeletal: Within normal limits
Other findings: None

1. Left apex peripheral zone PI-RADS 4 lesion.
2. No evidence of nodal metastatic disease within the pelvis.
3. No suspicious osseous lesions.
4. Suggestion of chronic bladder outlet obstruction.

The PIRADS 4 is probably going to be the main thing talked about. Usually calls for a biopsy.

Have you had problems peeing or with low flow? The bladder obstruction listed sounds like it could be problematic. Definitely ask the doctor what he thinks about it.

Looks like they didn’t find any spread. That’s good news.

@jeffmarc Have no problems peeing, and strong stream. I was looking at that as well, so im sure it will be discussed tomorrow. yes, very thankful no spread has shown.

Well the urologist visit was kind of a relief. He did say we will do a biopsy with the MRI image, and it looks like it could be around 50-60% cancer, but its in the very early stages, but the biopsy will tell us what we need to know. If it is cancer, then i could do radiation or surgery, but confident of a 100% curable. Im relieved for the moment. Still not looking forward to the biopsy..Urgggg..lol
