Tobramycin and psuedonomas
I find this platform so interesting and helpful.
Any opinions or experience on tobramycin and the treatment of psuedominas.
I am trying to compare treatment side effects vs continuing airway clearance with albuterol and 7% saline.

I was on tobramycin for 10 years. I stopped when my infection became resistant to it. However, until then, it worked well. I preferred the form that came in podhaler form, but when I went on Medicare, the price became prohibitive, so I had to switch to the nebulized form. Both worked equally well.
A chronic infection developing resistance to an antibiotic is a risk you take with antibiotics, but I did not have much of a life until I started taking it, so no regrets. I am now on another antibiotic that seems to be working.
What are you now on? 10 years is amazing for the Toby to have been of help. You are very fortunate.
Zythromycin. 3x/wk. I am better than I was before I took it. For the first three months I was taking it, I felt like I had no lung disease— better than I can remember feeling in nearly 20 years. Then pollen season hit and it was "here we go again," but I am better off with the zithromycin than without it. I also exercise a lot, which I am sure helps.
Hi spectrumcyclist. Just to be clear, are you taking Azithromycin? My pulmonary doctor told me not to have another doctor prescribe it as it is the "go to" med when I get in trouble with my lungs. I have bronchiectasis, Mycobacterium unspecified and colonized pseudomonas. I've been on tobramycin off and on for six years. When you were using it for the 10 years, was it twice a day and did they give you breaks off of it? I've just been put on it once a day. I thought that was odd but he wants to try it for 2 months and see if my pseudo improves.

I had some difficulty with inhaled tobramycin. It was difficult for me to accomplish the prescribed 4 weeks of the Toby. It constricted my throat and made me cough like crazy but I managed 2 weeks. It seems that many can tolerate Tobramycin. I was also prescribed Cipro for 2 weeks at the same time. My pharmacist told me that Cipro was a black box drug and was a drug of last resort. (Especially do not exercise while on it) I will not do the Cipro again after reading about side effects and having some myself. That combo, though, was helpful but did not eliminate the Pseudomonas. I believe that they are too colonized to eradicate at this point.
My new doctor suggests the next treatment would involve azithromycin. It seems that he is in no hurry to treat. I currently nebulize 7% saline 2x daily and do airway clearance, and I feel pretty good. I am in no hurry for more antibiotics.

Cayston is my miracle drug to inhale now. When I alternate with inhaled Tobi my sputum goes dark yellow in a day or two, my all over achy pain fibro kicks in and I feel like crap until the 28 days are up and I can use the cayston again. I have only been using the cayston again since this morning and already my sputum has gone clear!
you could ask your pulmo about 28 days on Cayston and then 28 days off of anything. Or 2 weeks on and 2 weeks off.
Tobi does not make things worse and my dr thinks it may be helping a bit.
i must have misunderstood your post.
It just does not seem to be helping but he says I cannot live on cayston all the time or I will become immune to its effects.
