TKR, Synovectomy, Lateral Release and still having issues

Had all these procedures over the last 20 months and still having what appear to be soft tissue issues and constant effusion. TKR was October 2024 and LR/synovectomy was December 2025 during which extensive scar tissue was removed. All imaging says the implant is fine but now the doctor thinks the issue could be related to the plastic button on the patella which i am skeptical of. I have tendinopathy in the distal quad tendon as well as the patella tendon, fluid in suprapatella pouch and noticeable plica/crepitis above patella. Going down stairs feels like i have gravel behind patella and above it. I continue to have fluid drained periodically which has been tested and does show to be "cloudy" indicating minor bleeding and some particles. He says he can go in again arthroscopically again and do lysis of adhesions and if patella scarring is the issue he would then cut me open and replace it. I've gotten second opinion and that guy said there was nothing he could do for me. I am at my wits end with this.
