Symblepharon: try to cope or surgical removal?
I’ve finally been diagnosed with a symblepharon in my left eye, more than 4 years after the procedure that apparently led to its formation. In 2021 my longtime ophthalmologist removed what was described as a benign granuloma from near my left tear duct. The minor scar seemed to heal but within a couple of weeks my eye started becoming highly irritated. I had moved across the country so couldn’t return to the same ophthalmologist, but the first one I went to said it was dry eye and prescribed prednisone drops and omega-3 supplements.
After a couple of years during which sometimes it felt better and sometimes it flared up with a great deal of inflammation, I sought out a couple of other doctors, who said my lower eyelashes were turning inward, causing the irritation, and that the solution was to remove the lower eyelashes, including the follicles.
I had that surgery in January, which seemed to help quite a bit, until scar tissue in my lower lid started becoming more painful and the lower eyelid rim, which had a trench dug into it to remove the lash follicles, now is thicker and also pressing on the eye. A week ago I found an eye surgeon who told me about the symblepharon condition, which she said is more painful because the buildup of the lower lid scar tissue has closed a space that should be between the inside of the lid and my eyeball, but the scar membrane is sticking to my eye surface, causing pain. She said I have the choice of trying to maintain through lubrication, minimizing the sticking as much as possible, or she can surgically remove some of the scar tissue to create the space needed.
However, she said a lens-type device would be inserted in the underlid to hold the space during healing and it would likely be annoying and painful. In the week following that day’s examination, when the lid and the tight scar tissue were pulled open for inspection, I’ve had some of the worst pain I’ve ever had in that eye, and it’s just beginning to feel bearable to open that eye again. This doctor said it is not a blinding condition (although I've read if it gets worse it can become so), but my left eyelid has been so sore and extra light-sensitive just since that bit of manipulation, I often had to keep it closed. Although I used to wear contact lenses, I have a feeling this “lens” would be much more intrusive and painful.
I also cannot afford to miss many, if any, days of work, which (of course) is the most eye-straining type: hours staring at a computer screen. I’m freelance, I don’t get any time off; if I don’t work, I don’t get paid. It’s been hell this week while my left eye was so sore, reading documents on the screen with just my right eye, which is now becoming more tired.
So I’m going to try the lubrication route first.
My questions are:
1. Does anyone have a similar symblepharon experience and what have you tried to do about it, with any success hopefully?
2. In trying to lubricate either for this condition or dry eye (the doctor said I do need better tears distribution, which may help), what are the best solutions? Restasis? Systane Complete or other versions?
