Stage 4 Prostate- started radiation

Okay year ago PSA 25, since then PSA .04, SUV's down for prostate, C7,scapula after radiation. Getting Lupron, Xgeva, abiraterone, prednisone. Not done L1 yet because of benign masses-kidney. Onc wants totake masses out. Uro want to observe. Radiation Onc wants to see what they decide before starting radiation on L1??? What should I do?

I had my L4 zapped three years ago. Seems to have fixed the problem my PSA went down to undetectable. They did notice my L4 at least a year before, but the drugs were keeping me undetectable so it wasn’t urgent yet. L1 is Close to the kidney so I understand why they want to hold off.

Are they considering a partial nephrectomy. Are they hoping it will shrink down so they don’t have to do the surgery, or at least can delay it. How is your glomerular filtration rate? Is it under 60? That gives you an idea if there’s really a problem.

Well my EGFR at my last blood test (4/24/26) was at 82, ( since I was diagnosed 77,69,87,84,89,88,now 82)but my ALT (388), AST (130), and ALKALINE PHOSPHATASE (344) were extremely high. Oncologist wants them do the partial or complete removal so that "we don't have to deal with it later". The radiation oncologist wants to continue radiation on L1 after they decide what to do. JUST WAITING FOR NEXT STEP-TAKING LUPRON, EXGEVA, PREDNISONE, LIPITOR daily.

I’m a little puzzled.

The ALT tests liver function normal is 0 to 47. Your seems to be excessively high, but you didn’t mention a liver problem.

The AST also tests liver function and 10-40 are normal numbers

ALKALINE PHOSPHATASE Also tests for liver disease and bone issues normal is 37-117

So all your liver function tests, show way out of range, but your kidney EGFR test is a little low but 90 or above is normal, but under 60 is a problem. As your age 82 could be normal.

Are you out of the United States and maybe they use different measurements?

Your liver results seem to be the real problem, do they say they are caused by your kidney problem? This sounds really intriguing. Is removing a kidney supposed to stabilize the liver results?

Nope here in Downey, Ca. So far there has been no mention of any liver question. I even asked my Oncologist and he just said probably inflated because of the different medications for now. I just did an ultrasound on my kidneys this week. They have done a biopsy on the kidney and that's when they said-some small masses but benign for now with potential for malignancy later! So far my liver has never been brought up. Even with the last results nothing was mentioned I brought it up?

I have a mass in one of my kidneys as well. It has grown larger over time, from 3mm in 2020 to 8mm 9/2025 but they just say it’s not a problem, benign simple renal cyst. My blood test results show it’s not a problem.

How big are yours?

@coachsanta Orgovyx alone raised 3 of my liver enzymes and with the meds you are taking, it’s probably the cause.
