Somatic Testing

Posted by johndavis60 @johndavis60, Jul 29 5:33pm

My MO ordered a somatic test. I already had a germline test and showed no mutations. Anybody here have somatic testing and did it change specific treatment plans or did it just confirm things that were already known?

Maybe ask about doublet therapy as @mjp0512 is having. All those negative findings may indicate the need for a more aggressive approach.

Thanks Phil, MO said something about Abiraterone + Prednisone at our 1st appt the other day. I think she is waiting for foundation One test plus I have a PSMA PET scan today. I’ll bring this up again next Wednesday (8/6) at our next appt where I also begin ADT Firmagon.

Good! Also, just read some VERY positive things about Darolutamide and how it may obviate the need for chemo in many cases and avoid triplet form of therapy. I’ll see if I can post a link.

Somatic findings don’t always stay around, You could have BRCA2 Show up in your somatic test today, a year from now, it might not be there.

The thing is, if it still Shows you have somatic BRCA2 When your current treatment fails, you could start on a PARP Inhibitor.

My GU oncologist has been working with me for about eight years. Even though I am BRCA2 She has had me take other drugs before the PARP inhibitor. It has major side effects so she wants me to avoid it until it is essential. When Darolutamide fails me I will go on a PARP inhibitor.

So what your doctor said makes perfect sense.

Is BRCA1/2 found through somatic testing or genetic testing? If through genetic testing, I’ve heard that it will always be there, and never need to retest. If through somatic testing, then it might change over time.

I think you have it right Brian.

BRCA2 can be found in the hereditary genetic test, In that case, it would be in the somatic genetic test as well. Both of them are genetic tests.

People who do not have BRCA2 in their hereditary test, can have it in their somatic test. Over at Ancan.org we’ve had a few people over the years that have had this occur.

Apparently, my urology oncologist stays pretty current. He started me on this therapy 3 days after this ASCO meeting. I may have to keep him. 😁

Thanks Surfer, and you do remember correctly. Also cribriform, IDPC and Gleason 4+5=9. I’m hoping that Firmagon and radiation will help now that I have my prostate out, I have an PSMA pet scan tomorrow and my psa is creeping up at 1.52. Also hoping for no more bad news! 😉

Thanks Surfer!!
Image doesn’t look that much different from the PSMA scan I had on March 27. Before my prostatectomy and lymphadenectomy. I was fearing the worse (as I commonly do). Still waiting for report. I’m hoping the white parts between my kidneys and bladder are parts of my ureter.
