Should I go on Envenity, Forteo, or wait?

@kfhoz I am on 2,5nd month on the anabolic drug Teriparatide/Forteo. My endocrinologist measured only P1NP ( I am not sure why not CTX as well), and it shows a great increase in P1NP. At the time of my vertebral fracture in February, my DEXA was in osteopenia, and my bone marker ratio was 182; that means my bones were probably in an anabolic (good) state, but my endocrinologist put me in the clinical diagnosis of "fragility fracture osteoporosis".

I am still on the fence myself. I am getting to be an expert in putting things off that I should do. I'm 69. My 2024 scores were left femur, -2.8; left total hip, 1.6; left total spine, -2.3. My score for 2026 is unchanged except the spine score decreased to -2.8. My endocrinologist prescribed teraparatide but I've never liked taking medications and I just can't make myself start the teraparatide.During my pregnancies when I was young, I wouldn't even take Tylenol! I may have to get an appointment to talk to the endocrinologist at length about this because I am so apprehensive about it.

erinz, it is scary.
Forteo is the least scary of all these medications because you can quit it any day you please without negative consequence from taking it. It leaves your body quickly usually within four hours, so if you even have side effects they resolve quickly, usually within those four hours.
It works by increasing the cells that build bone (so called anabolic) it also increases the cells that remove older fissured bone, so that the new bone is lain over a clean surface. It is the way our bodies naturally preserve the strength of our bones.
If you were in a more risky T score zone, I'd suggest that you consider Evenity. Evenity lays down bone the fastest.
If you decide to take Forteo, you might ask your doctor for bone markers CTX and P1NP before and a couple of months after beginning Forteo. The markers will tell you if the medication is working.
Remember that it is your decision.
I hope you join me. I'm on extended Forteo. I love this drug.

@gently Yes, I agree for the greater part. As you know, Tymlos has many similarities with Forteo. I think Tymlos deserves to be named next to Forteo as the least scary medications.

And yes, I've been on Tymlos for 5 weeks now and at least feel fine (and after 3 months of use, the P1NP and the Ctx values will be measured, so I'll have a first indication of whether it actually works for me).

Tymlos may be a better medication than Forteo. (I'll never admit it.) It seems to have slightly more impact on bone mineral content and is less likely to cause hypercalcemia.
I'm glad it's okay for you-- full dose and no side effects?

if you do decide to take teriparatide there are a few things that can make it a little easier. But don't get pressured into taking it.
I try not to express my enthusiasm for the drug. because every person should look carefully at all the options and decide according personal need.

@gently Yes, full dose and only negligible side effects. I was rather scared before I began injecting, as I'm small frame and my blood pressure is a bit low.

Thanks to your advice, I knew why drinking lots of water is so important. If I forget to drink enough, I get a bit of a headache, palpitations and nausea. But when drinking enough water, I'm just fine.

Tymlos doesn't have to be kept in the fridge as long as the room temperature is 25 degrees centrigrade or less. That's very handy with traveling.
