Should I go on Envenity, Forteo, or wait?

Posted by lori098 @lori098, Sep 3 4:11pm

I am 62 and have a -3.1 in my left hip. Other scores are osteoporosis, but not as low. Dr recommend recommends Envenity as a first choice, Forteo second. I don’t take any other medication but a year ago I was on fossamax for 4 years which didn’t help. I eat right and exercise and I’ve been doing that for 25 years. I am told I should try to build bone now before I get a fracture . Also, I am going to need more back surgery eventually and I am told I should try to build bone before the surgery, but I am very nervous about taking medication. Any thoughts?

@boneoptimist the problem regardless is after the bone builder you have to go on bone savers. I will be faced with this in less than a year.

@ccps101 , I will be faced with it in a year as well. My Dr said that they are finding that lower and fewer doses of the savers work just as well. My crazy plan is to get to normal bone density in the next year, stop Tymlos, keep up the nutrition and exercise part, then ask to monitor with Dexa at 6 month intervals, if I can keep the density steady, i will skip the savers for the foreseeable future.... I really, really don't want to be on them. Optimistic, by hey , that's my name....

sounds good boneoptimist, but I could wish you were keeping closer track with bone markers CTX and P!NP.

@gently , the Dr has me scheduled to recheck P1NP again in a few months to make sure my body is still responding to Tymlos. It was 155 right after I started Tymlos in Oct 25, and 250 in January 26 and 244 a week ago. Not sure how good that is, but the Dr was satisfied.

@boneoptimist
perfect. CTX is a measure of bone dissolution and P1NP a measure of bone acquisition. We like to look at them in relation to each other to be certain that we are gaining more than we are losing. 400pg/mL to 155/mcg/L 821 pg/ mL to 244mcg/L both sets look great, interestingly. You may have had a rapid response to Tymlos and/or you may have been building bone when you began the medication. Either way Tymlos has greatly increased your bone turnover with the advantage of bone building over bone resorption.
You might look at the Lucas video. https://www.youtube.com/watch

@boneoptimist Link from @gently did not fully work, but she may have meant this one:

kfhoz, thanks for covering my mistake.
I meant to link this video https://www.youtube.com/watch.
All of his videos are well worth listening to, unless you buy something, then it's a bit of a scalp. Lucas is elevating the discussion of osteoporosis through the science, changing the quality of care for women through women.
I would edit his initial statement about education and prevention and all of the ads, but

@gently , And I had been exercising heavily and changing nutrition and supplements for 1.5 years before starting Tymlos and getting those first blood test numbers. Bone density was already increasing at that point, before the meds.

boneoptimist, your first set of bone markers show anabolic growth. The second set is more assuredly remodeled bone with higher numbers indicating accelerated growth. I find both remarkable (ok, enviable). I want mine in the second set model.
I could wish for a set of markers before your concentrated efforts.
You've blessed your bone.
