Sharp pain big toe

Posted by Soledad02 @soledad02, Nov 9, 2025

A few weeks ago I started noticing a pain at the end of my big toe. The pain goes away for a few days then returns. It's not irritated, not red. I thought maybe my walking shoes were too narrow. I bought a new pair in a wide. Last night the pain returned with a vengeance. The pain is just at the end of by left big toe, very sharp almost like having a needle injected in it. I tried Voltaren (arthritis cream), and also a compounded pain med my pain management doctor prescribed when I get pain in my heels (arthritis). Neither cream worked. Could this be arthritis, if so, what might work. If pain persists I'll see my podiatrist.

@closer0043 it’s crossed my mind. Did you see a doctor to get diagnosed? Dud you find anything that helped you?

This has started happening to me too! What the heck. I’ll be watching TV and I feel sharp pain, kind of pinching, kind of like a needle going into top of my big toe. I will be watching to see what any finds out from a doctor. I need to get to my podiatrist for something else and will ask her. Gosh, I thought it was just me having some weird - who knows what.

@willow5 my pain went away night before last and still pain free! I was ready to see my podiatrist but thankful no more pain. I’m fearful it’s going to return. Crazy because I’m not sure what’s causing it. Good luck and keep us posted on what you find out.

Please do see a podiatrist. It could just be arthritis and if topical creams are not soothing, then a injection would do the trick. Gout will show up flaming red and swollen Speaking from experience :).

I experienced this through my 20s/30s. I was evaluated by a physician, and he found that I have a bony impingement at the top of my big toe joint. In lieu of injections or surgery, I stopped wearing heels and any other footwear that aggravates it - I am now pretty much symptom free in regard to the pain that you describe as "just at the end of by left big toe, very sharp almost like having a needle injected in it." You may also be experiencing an impingement (either congenital or arthritic).

@derrsk11 thanks for this info. I'll see my podiatrist "if" the pain returns. I'll keep this information as this could be what I had.
