I'm not familiar with Mayo Clinics financial arrangements or what the qualifications are.
I'm sure someone more knowledgeable will be along.
I did see this,

If you then click on the Financial Services button, you will find a cost estimator tool and phone numbers to contact, depending on which Mayo Clinic location you wish to visit. You may call the number associated with the location you're interested in to ask your questions as a patient coming from outside the United States.
