Prostate size 5.0x6.0x6.1 (95 cc)
Transitional zone: hyperplastic changes
1 lesion, mid gland, 0.5 cm., t2: hypotension t2 signal with question of mild heterogenity, does not about the capsule.
PI-RADS 3.
No enlagement of lymphnodes, normal bladder, no free fluid.
The one lesion found is possibly an extruded hyperplastic nodule, but difficult to say due to small size.

Did the MRI report mention anything about extracapsular extension, seminal vesicle invasion, or perineural invasion?

You have a large prostate. That can frequently cause higher PSA numbers. A PIRADS 3 Is a real questionable lesion?. It may or may not have cancer, More likely not. The thing is, they can find cancer in other areas of the prostate that don’t show a tumor.
The PSAD is very low unlikely to be cancer. What is your total PSA? Because of the size of your prostate, it could be higher than normal, But it doesn’t look like it based on your PSAD.
You can always get a PSE test to see if you need a biopsy. It is very accurate and can tell whether or not there is cancer markers in your bloodstream. I’m including a table that shows you the different tests that you can get to tell whether or not you have a likelihood of prostate cancer.

jarvis pelvic area normal, seminal vesicles atrophied, lesion not touching capsule, in fact, may be close to transition zone, and I think she's (Wendy Tien MD) suggesting it began in the middle of the prostate and is a bph nodule that moved into the periphery. No mention of other areas outside the prostate, but Im sure they would've been mentioned if anything untoward was there. My first psa was 11.80; retest was 5.20, which is a return to my baseline.
@jeffmarc my psa retest is 5.20, this seems in line with my previous 2 yearly tests, both 5.17. I was convinced I had bone Mets but the mti with contrast most likely would have found them since my urologist asked the mri techs to look at my pelvis and lymphnodes and that general area, too. If it is cancer it is probably small and local. Thanks for the information, very kind of you. I was going to ask the doctor, in case he still wanted a biopsy, if I could get a pse test before they started sticking needles in me, was also thinking of getting a GU subspecialist to look at the mri, maybe getting it reduced to a pirad 2. Leaning heavily towards a psa retest in 6 months, that is, without doing anything just yet.
