RBC & Hematocrit increase after lowering HRT after heart attack
I'm hoping someone can point me in the right direction as to what to look for as it relates to my husband's recent bloodwork.
In August of 2023 he had a massive heart attach. Two stents were placed in the posterior and anterior left arteries. He had an eighty percent blockage in one and one hundred percent blockage in the other. In addition to the stents he has PVC and high cholesterol.
There is a family history, on his father's side, uncles and cousins, of heart disease, high cholesterol, and stenting.
The PVC are currently being controlled with Metoprolol.
He has some plaque in his coronary artery and recently had an abnormal ECG. His cholesterol is being treated with Repatha and Zetia.
He has been on HRT for many years and over the last seven months has drastically reduced his HRT to help bring his RBC, hemoglobin, and hematocrit lower. Without getting into exact numbers he has reduced it by three fourths of his original prescription. He also takes Anastrozal to keep his estrogen from getting to high, which causes him alopecia and cystic acne when it's not controlled.
His recent bloodwork, January 15, 2026, came back normal except for his RBC, Hematocrit, and Hemaglobin. Those numbers actually increased from his last bloodwork in June of 2025. It isn't making sense to me that after decreasing his HRT by three fourths that those three numbers would increase instead of decrease. We believed that his HRT was contributing to those high numbers. His blood pressure is controlled, his testosterone total and free are very low, and he is still taking Clopidogrel, to prevent clotting.
Now here's a monkey wrench. We live full-time in our rv and travel to see our children and grandchildren. He has a cardiologist in California and a cardiologist in Virginia. The doctor in California, that's where we are currently, basically blew him off. Ordered bloodwork and told him follow up in a year. Also told him to stop taking the Clopidogrel, not wean off just stop, which we refuse to do under the circumstances.
The cardiologist in Virginia hasn't had an opportunity to weigh in yet. I'm a patient at Mayo for other issues and I've been trying to convince him to get a doctor at Mayo.
Are there any men in their sixties who are currently on low dose HRT, had a heart attack, and have issues with Hematocrit, Hemaglobin and RBC ?
If your counts are under control what have you done to get them there?
If any of the cardiologist follow these boards I'd appreciate as much insight as you can based on this limited information.
Who would you recommend he make an appointment with at Mayo?
I'm very very concerned for his health and well-being at this point and I don't feel he's received the level of attention that his situation may warrant.

I can't help with the problem as I don't seem to need HRT and have no problems except incipient hypertension and atrial fibrillation, the latter blocked via a catheter ablation. However, I do appreciate that you have potentially cross purposes in the way of advice or prescriptions. Yes, by all means, get a third opinion. Maybe two of the previous opinions will match and you'll feel better about following them.
Personally, as long as hecan walk and don't fall or whack my elbows and hands on things and cause contusions or subcutaneous bleeds, or cut himself regularly with kitchen knives, he may still be at some risk for clotting and therefore a stroke. So, some kind of 'thinner' or novel anti-coagulant might be a very smart idea if you'd both like to be around each other for a while yet. And be able to walk, hold hands, and communicate verbally.
