Orgovyx and the timeline for return of normal testosterone level

Posted by rmp321 @rmp321, Dec 19, 2023

I’ve been taking Orgovyx since 11.09.23. How long does it take for Testosterone levels to return to normal once I stop taking this drug? What side effects have you experienced with this drug?

That reflects my thoughts as well. Let the Testosterone recovery time be factored in with ADT (stop ADT earlier if recovery time is longer). Get good scans (maybe multiple types), test & understand your particular cancer cell genetics; then see if you can go after them with targeted methods and/or multiple treatments. Never just blindly accept treatment without understanding the reasons and options - questions like these are great! Thanks,

27 teats after cryo to remove prostate, Dormant PSA spiked to 45. August 2021 started Lupron injections and eventually had 2 rounds of radiation to treat clones. PSA came back and went to docetaxel August 2024. Finished 6 cycles last week. Currently taking orgovyx

It sounds as if your persistent PSA requires continued treatment. Not an oncologist so can’t say what next steps should be done, but going off ADT could be premature….JMO

I was only on ADT for 4 months. My first testosterone test showed 1.62. My second on Dec 24’ showed 401 which is mid normal range.
My libido is still an issue and becoming frustrated although I am starting to get better blood flow and a bit better desire after taking Cialis daily. Most of the other side effects have subsided.
Hot flashes
Weight gain
Moodiness.

How far apart were your first and second T tests? 3 months or 6 months? Thanks.

rmp321,
I found this on the orgovyx site. "Cumulative probability of testosterone recovery to 280 ng/dL at the 90-day follow-up in 184 patients who completed 48 weeks of treatment and who did not receive subsequent androgen deprivation therapy for at least 90 days after discontinuation."
I've been looking for a chart. And wonder if a person can take testosterone injections to speed the process. Or cover the interval or if it would be contra indicated with the prostate cancer history.
So it is the second to the last chart on this site. https://www.orgovyxhcp.com/efficacy/ Not exactly what I wanted to read.

I'm currently on Lupron and will ask to switch to Orgov. for my second 3 months of 6 months treatment. Orgov. has a faster rebound speed than Lupron. I would consider 184 within 3 months positive and personally won't touch T supplements as they are psa rocket fuel. My therapy start point was T-250 which is considered low T and was a huge win for G-9 Grade 5 cancer.

I'm told recovery depends on a number of factors including age. I'm 72; ended 4 months of Orgovyx in January; T has been slowly, much slower than I expected, creeping up and is now 250. I still have hot flashes; moderate ED and extra belly fat. I met with a member of my medical team yesterday who said, 'more exercise" and when I said that I'm doing 30 minutes per day, she said "do more" and more diet control...ugh! She does not believe that I'm candidate for T supplement and that it would increase risk of growing cancer cells. My PSA is undetectable so the T issue is the price for that wonderful news.

I doubt any Dr would actually want us doing T therapy as it is cancer fuel. 30 minutes is considered minimal exercise. My routine at 71 is a 45-minute spin class with approx.. cal burn of 450 followed by 45 minutes or so of weightlifting with total 550-600 cal burn. And if weather allows, another 450-500 cal burn of bike riding either on mtb trails or neighborhood trails. My normal calory intake is between 1600-2200 per day and as one the is prone to weight gain but recently losing 32 pounds I monitor my calory intake and burn using the Noom app and a heartrate monitor. My goal is to attempt to minimize weight gain during ADT. My doctor wants me to keep officiating lacrosse during treatment. I looked at him like he had a third eye lol. Said cardio problems are rare on short term ADT. Best wishes on your recovery and journey.

My lead RO referred me to a (fertility specialist) urologist saying there would be little risk, given active monitoring, for T supplements. He's nationally known and has 40 years of experience, hence, I trust him more than most. Surprisingly, my much more conservative younger RO Dr. made the same recommendation saying, "several of her patients have done well with the supplements" and a third RO concurred. I'm not rushing into anything and will consider all these doctor's opinions and well as yours.

I've had 4 continued ADT (Orgovyx 4 months) side effects after one year: hot flashes, moderate ED, lower strength/energy, belly fat. I also exercise daily and have a personal trainer to help out. Dialing back the side effects is highly desirable for me, if I don't take on unacceptable risk. I realize we are all different and our risk/reward approaches should take that into consideration. Postings and discussions like this help me and others evaluate what is best to keep PCa in remission and enjoy our lives as much as possible.
