Oral Lichen Planus - my new dental treatment plan...

I am so frustrated with my current dentist. I am loosing teeth - six in the last year and another currently needs to be pulled - also have a partial they keep having to adjust and/or remake as the anchor teeth are now gone. I requested he refer me to "someone else" either another dentist or maxillofacial specialist, etc. I told him I was willing to travel. He referred me to a periodontist in a larger town 20 miles away. My first visit was yesterday and I'm hopeful. I'd previously been told that my gums and/or bones would not be supportive enough for implants or even dentures. This periodontist said one thing that he feels would work for me is a denture and use perhaps three implants to secure the the denture in place. He wants to first get my OLP in better condition and then do cleanings every 3 months. I'm to use Dexamethasone rinse four times a day. He also agreed that the one tooth that was questionable with the other dentist is loose and needs to come out. Luckily it is not an anchor tooth. Are there any of you that have been in this situation, i.e. have OLP, weak gums and bones that have had dentures with implants? Appreciate the replies.

I also used to wear a full mouth guard. It was not tight. Because I lost so many teeth I did some major trimming and it only is positioned in the front of my mouth. It's not a perfect situation but I'll use it until I know the final outcome with my situation. Thanks.

My bottom front teeth are all loose. Because I know a bottom denture can be hard to keep in, and expensive ones are really the only way to go, and in order to hold onto them as long as I can, my dentist glued a brace behind them.

It's literally a small chain they use for people after braces. It's attached on the back of my front bottom teeth, It attaches (glues) the loose teeth to each other and then the chain connects to stable teeth on either side. I think they have even started to tighten up again as they are not at all wiggly or they are definitely less likely to fall out. It's kind of all or nothing. and they are now attached to strong teeth. Nothing shows from the front.

It was my hygenists idea as she had seen a dentist do it once. She called the new to me denist in, and talked to him and he said it might work. That was two years ago. I think it was $700 for him to put it in. I'm sure it was over an hour to get it right. I brush often, always after eating and waterpic to keep all the little places clean.

Now, my bottom front teeth aren't all so great as I've been a grinder for years and that is what is making me comment on your post. I wore a tooth guard also. I still ground my teeth and instead wore a hole in the guard.

My thought for you is if it's only in the front of your mouth, and you are still grinding, are you putting more pressure on just your front teeth and is that worse? I don't know, don't take this as anything but a thought and a bit of mechanical thinking. Will your front teeth get looser faster?

A full mouth guard at least disperses the power/tortion of the grinding to your whole mouth and not as central as just having all that power on only your front teeth. I always thought grinding was all of the reason for my loose teeth. I have had OLP for more than a decade but didn't know it could affect bone. I also have CRPS which affects teeth/bone too. I need to hold onto the front bottom teeth as long as I can. Bottom dentures are hard esp if your bone is kind of collapsed. Chewing is esp important as we age of course, getting esp greens into our systems. I did get 4 crowns on my 4 front teeth 10 years ago and wear a partial on top that connects to them and the other few teeth "bone worthy" to hold it in.

Nothing makes you feel better than being able to have a nice smile. I laugh more since getting the crowns on top. Teeth problems can affect our self esteem, and even our desire to leave the house and be seen.

Thanks for the reply minnesota10. Your question, "What is going on with your bones?" is one I've asked each medical person I see. Other than "take calcium" is the only response I've received. I began getting bone density tests in 2002 and I'm osteopenic. It's like riding a downhill runaway train that I can't stop. I take it you're a Minnesotan--I married a guy from Austin. Best to you.

Have you seen an endocrinologist? They are the ones that really helped me to get my bones healthy. I was just back there after many years and they gave me a new calcium to take. It’s a chewable. I wish you the very best in 2025

Thank you for this information. I did look hem up and got a lot of good information, but no one really in my area who specializes. My OLP is not flaring up inside my whole mouth right now, but there is always this white spot along the inside of my right cheek that is raised up, tender, and painful when eating anything sharp (like crackers), acidic or spicy. And a spot behind my left wisdom tooth that is also tender and painful when eating or brushing. Should I seek treatment now or wait until a flare up so the doc can see the full extent of the problem? Sounds like a silly question, but the ENT doctor I was referred to told me to wait to come in. Do ENT's know anything about OLP?

@metta That is not a silly question - in fact no questions on any Mayo Connect threads are considered silly as each one reflects a genuine concern. And if those spots hurt, I would consider that to be a flare!

I have experienced those spots on my inner cheek and behind my wisdom tooth too, I just apply Lyderm gel to the sore spots with a QTip and that eases it. I don’t know about the States, but in Canada you need a prescription for the Lyderm gel.

Hello again all, I have currently been taking Orajel mouth wash for OLP mouth sores swish twice daily and a new dental paste called Triamcinolone Acetonide Dental Paste, USP, .1%, for topical use in adjunctive treatment of oral lesions. My pharmacist told me about it and I apply it to my mouth sores and cheek linings twice a day. I've been using it for about 2 weeks now and it has really helped with the sores and pain. It's by prescription and it's a pretty tiny tube - 5 grams, but it seems to be working so I'll put this information out there so if you like you can check into it and it might help bring you some relief. Hi Metta. My dermatologist prescribed Triamcinolone Acetonide Dental Paste, USP, .1% years ago. It does help a great deal. I apply at night in hopes it will stay in place longer. You are so right about the size of the tube--it tiny. Thanks for sharing. Faye
