Sorry you had to dive into the club. When is your PSMA PET scan scheduled?

Get them to submit your biopsy tissue for Decipher right away!

The Gleason 4+4. is really the one that makes it more aggressive. The lower numbers do not really matter for treatment decisions.

Perineural invasion is very common, It’s one of the few things you don’t really have to worry about.

4+4 is very treatable with radiation or surgery.

Sorry to hear that you were given membership. As has already been said, with a 4+4, you should get a psma and a decipher. I would also submit the biopsy to Hopkins for a second reading. It seems there is no mention of cribriform or intraductal, which is good.
Many 4+4 at biopsy get downgraded at pathology. That happened to me, from 4+4 to 3+4.

Sorry you have those results, but at least no cribriforms or SVI. Also, really not that many positive samples. I don't think it looks bad for biochemical recurrence (BCR)? However I am new at this, but that is my thoughts.

Likewise, i had one Gleason 8 (4+4) at biopsy and it was downsized to Gleason 7 (4+3) at post surgery pathology. My initial biopsy had three 6’s and 2 favorable 7’s and that one 8.

Another one who was downgraded after pathology (3+5 intermediate unfavorable downgraded to 3+4 intermediate favorable), so take a breath, hold it, and let it out nice and easy, you’ve got this.

Welcome to the club (that you would prefer not to be part of I'm sure). Hopefully the interaction on the forum will help you to navigate this journey. You are not alone.

I'm sorry you had to join the club. This group is amazing and I'm lucky to have found this forum I don't think I could have gotten through my Cancer Diagnosis/Surgery and Journey without this group. Keep on posting here! We are here for you.

You are in a great club of people who really do help others. I have been involved for almost 3 months and can say that the advice and quickness to respond has been incredible. I am sure that without this group, my stress would be much higher. It’s kind of like crowd sourcing; you can elicit stories that are tailored to your concerns. Any who, sorry about your results and wish you the best…take care
