Need to Make Decision between Trans-rectal or Transperineal Biopsy
Hello, everybody,
Please advise on what biopsy to choose: trans-rectal or transperineal.
Leision 1, PI-RADS 5, 2.51 cc centered at the anterior fibromuscular stroma at mid gland-to-apex and crosses midline and there is anterior capsular bulging. ADC signal 596 suggests that clinically significant cancer is present.
Lesion 2, PI-RAD 4, 0.11 cc at left horn peripheral zone mid gland.
Please advise what biopsy is better. The hospital Nurse Practitioner is pushing for trans-rectal because according to her there is a longer waiting time for transperineal.

At Mayo Jacksonville, the norm is trans perineal for biopsies. Transrectal is done when having anesthesia is an issue or there are other issues. The transrectal is done in an office setting and the transperineally is done in surgery setting with anesthesia specialist. However, this was Mayo Jacksonville and 2 years ago. I do not have information on other facilities.
My Jacksonville urologist said to me that a urologist at Rochester had invented a method of doing trans perineal without anesthesia. That was almost 2 years ago. I preferred trans perineal but had to insist on it.
I am glad I did it transperineally with anesthesia. No anxiety, no discomfort, no stress. They used a method called MRI/Fusion to do biopsies.
