Montgomery Long term Trache Question

Does anyone have one of these? I had one briefly for 2 months but mm it came out when t4ying to sit the adapter inside it. The adapter also blows off with the speaking valve and HME attached when I cough hard. Has anyone else has this problem and what can I do about it? I felt like superglue it in but doctor said not too. It's hard to keep att a ching the adapter after blowing off.
