Maximum time I can postpone treatment after biopsy?
Hello everyone. I had a 12 core biopsy 12/5/25 which showed 2 cores on left side 3+4=7 , gg2 favorable. No cribiform detected, no SVI detected. 1 core showed a bit more #4 and more total % involvement, I have been on AS for a few years with a 7.5-8.5 psa and MRI showing nothing suspect. Pirads 2 . Mri was 3 years ago. I had kind of decided to have surgery when it came time to deal with the cancer. Now my dilemma: I have surgery scheduled for 3/10 and have been stressed about the timeframe of recovery. I farm and spring work begans in early April. I am 65 and know it will probably take at least 6 weeks or more before i can do most of the work required to run the farm. So that really isnt feasible with the march surgery. Other option is to wait and have surgery late fall this year. I have much more recovery time in winter months. Guess I am asking for opinions about with my biopsy results is waiting a risk not worth taking? thanks

After revisiting my biopsy results there was one sample that seems maybe more concerning. The left "base" two cores 3+7, 30% #4 pattern. 70% core tissue area involved. The right side showed no positive cores. One 5% core on left posterior. Ive been doing research but no where near as knowledgeable as some folks on this site. Thanks again for your input.

I think you meant to say 3+4 A Gleason 7. That really is low volume even at 30% of 4. Either surgery or radiation could very easily destroy or remove that. Some people have much worse, tumors inside their prostate.
I would not be concerned about this at all. The 3+4 also makes it very slow growing.

@colleenyoung I am meeting with my urologist on Friday to discuss some topics, Decipher test, Pet scan, possibly refer a good RO , and options to possibly mitigate/arrest the cancer if the psa and other indicators become more worrisome over the summer/fall until surgery or other treatment options can be started. I am in the process of doing research exploring all treatment options so hopefully be more informed to hopefully help make a decision next Fall.

If you plan on waiting a while and have a high decipher and Gleason score, then they went You to start on ADT, which will stop the cancer from growing and spreading. That way you can delay treatment.

1 ReactionYou should “start” to feel better at 6 weeks. If you’re driving a tractor or a combine, etc. get a seat cushion or two for the bumpier ride. And pack a bunch of diapers. With that bumpier ride you’ll be pee’ing a lot involuntarily.
