MAC Medication Side Effects

Posted by macsucks @macsucks, Jan 20 12:37pm

I am on the following and seem to be tolerating it well.
Ethambutol 400 mg. 4X Morning
Rifampin 300 mg. 2X Morning
Clarithromycin 500 mg. 1X Morning 1X Evening

Except: Mild stomach issue. Not really nausea or stomach ache. Just an underlying feeling of something different. More of somewhat of an awareness of my stomach other being hungry or full. Can't really put my finger on it. Justca different feeling. It does seem to be curbing my appetite and I have lost some weight. I usually put on 10 ponds during the winter. Just wondering if anyone experiencing anything like this.

@visions63 - wow. Big 3, plus another med, picc line and surgery, that’s a lot to deal with but knocking it out is important.

This is what my dr recommended: Fortify 50 billion CFU per day; or "Physician Choice" 60 billion CFU per day; or "Wholesome Wellness" 100 billion CFU per day; or Garden of Life Raw Probiotic 100 billion CFU = 1 capsule once a day with lunch.

I went with the Physicians choice which I could by locally and worked well for me.

Best of luck with all you have going on and good vibes for negative cultures so you can start the countdown to going off the meds.

@visions63 A bit of advice re PICC line: Have it inserted into your NON-dominant arm! Otherwise trying to insert the IV fluid into the line is very difficult if not using your dominant hand. I've had PICC lines several times (mostly for chemo), and I'm beyond grateful my first oncologist years ago recommended a non-dominant arm insertion. Carried out life like no big deal...

@rmoore901 Sounds like a knowledgeable physician to know those choices and amounts to take.

@blm1007blm1007 in this specific instance yes. That being said he never suggested the option of watch and wait, 3 day per week antibiotics, airway clearance/nebbing etc. I learned so much more from this site. The same Dr also left his practice while I was in the midst of treatment (which happens) and spent the last appointment telling me he was done and to pick a new provider on the spot so records could be sent. Also stated that all existing labs he had written for blood work, sputum etc were no invalid so it would be up to the new provider to decide on the treatment and follow up. Once I got in with someone else, she took me off meds and said she would not have started me on them so I went off after about 8 months (5 months after a bronchoscopy showing MAC cleared). On the other hand he did monitor with monthly blood work and referrals for vision and hearing screenings so there were some positives.

@visions63 My original treatment was Big 3 every day and nebulize Arikayce. Didn't tolerate Arikayce, so yes IV Amikacin and much better tolerated with only a bit of fatigue the very first week. Most days I forgot I even had the PICC line in except daily IV time. I've
been on Big 3 for 11 months now and am doing great in every aspect. My meds are timed for maximum absorption and virtually zero disruption to my very busy life! Lmk if you have more questions. Terri

@visions63 me. Except for a periodic "tired" day, I have zero side effects.
4:30A-Sunrise yoga/IV (only during summer 2025).
5A-Black coffee/ACT Neb 7% followed by 7-8 miles daily run (aka Add'l ACT & Mental Health Care).
730A-Breakfast
8A-4P-Living my wonderful full life of daughter, wife, friend, mom, grandma to 4 busy boys, tennis, golf leagues, and lots of volunteer work.
4-5P-ACT Neb 7%, Dinner.
7:00P-Ethambutol and Rifampin with FULL glass of water. Bedtime@8P.
2-3AM-Potty/Azithromycin and NAC with Vitamin C tab.
Also, I've always been an upbeat, positive, glass half full kinda gal, so this "little bump in the road" hasn't stopped me. Just simply tweaked my already very busy and full life. On my "fatigue" days is when I get some crocheting, reading, or baking done plus take a nap. I realize I'm blessed and VERY lucky 64 yo for having zero side effects.
