Welcome to Mayo Connect. I am sure you have already searched for an answer, but I did a little research as well.
Before Medicare will consider approving a drug, it must first be approved by the FDA for use in treating a specific condition. Ketamine is a Schedule III controlled substance that is FDA-approved for the induction and maintenance of general anesthesia. There are some references to a nasal spray for treatment-resistant depression.
I suggest you contact Medicare at 1-800-MEDICARE (1-800-633-4227) for the latest information.

@medicare94 you need to be more specific as to what is being treated. Ketamine comes I different formulations to treat different illnesses. Medicare does cover Ketamine for depression and PTSD just off the top of my head. It may also be covered to treat chronic pain but not by infusion.
