Kappa free light chain
So I’ve had high wbc and I had a ct scan. My ct scan showed enlarged spleen and liver. They sent me to a hematologist. She did tests to see if it’s possibly MPN so far everything came back normal but my kappa free light chain was a little high. The ratio was normal and lambda light chain was normal. What could this mean? I’m worried. I just had a baby a month ago and have two other kids. I’m hoping not cancer..

My oncologist has told me as long as you the ratio is ok not to worry . Both my light chain numbers are very high but ratio is within normal range
That is my understanding as well - although my understanding hardly substitutes for a qualified medical opinion. The absolute value of FLCs means less in terms of a possible plasma cell disorder than does the ratio. A number of things can cause elevated FLCs, but a blood cancer would generally cause an abnormal ratio - in other words, the increased production of one FLC over the other.
I have high Kappa and high K/L ratio. But my oncologist says it’s a type of MGUS.
I'm still awaiting an official diagnosis, but recent blood tests revealed:
M protein band 1 is 1.1g/dL
Free kappa light chain 246 mg/L
Kappa/Lamba Free ratio: 34.66
Leukocytes & RBCs & platelets all in normal range.
Is this anything like your situation?
I'm very nervous.
So all of my labs have returned normal or negative for signs of a cancer or infiltrative process such as sarcoidosis.
I guess the next step is to refer me to Hepatology for evaluation for possible biopsy and performing secondary blood tests. I’m still so scared…liver cancer?? 😭
Your M-spike is still good, mine is 1.2 and the oncologist says it’s okay. But did they schedule you for a BMB? They get ALOT of info from a BMB. My report was 11 pages long.
Thank you for the encouragement. No BMB scheduled yet. I just had the blood test done a few days ago as I have been struggling with gastritis and elevated liver enzymes for about 10 days. I had a CT scan that showed no evidence of cancer in my liver or other main abdominal organs, but this blood test has thrown me off. I don't think the M-spike is connected to the gastritis, which is getting incrementally better. I suspect that my primary doctor will be referring me to a hematologist for deeper evaluation of the M-spike and free kappa chains.

Normal is good, eh? Although there are many members who can give some great anecdotal information, you’re going to get you definitive information from your hematologist/oncologist. It is frustrating to wait and now that “My Chart” is available online, we are prompted to speculate on our own.
Have you gotten this referral and appointment scheduled yet? Many of us live quite well with MGUS if that’s what is eventually diagnosed. I remember those anxious days and how literally sick with worry I was. I hope this period is short for you. We’ve all been there. Will you let me know how this is going for you?
Thank you Patty. I have not yet received the referral for Hematology/Oncology since the blood test results came in over the weekend, and it is now just Monday. Yes, it is hard to wait. In my health plan I receive a notification each time a new test result comes in, and then I can immediately access the data. I likely see this well before my primary physician does.
Furthermore, I actually teach advanced courses in Immunology & Hematology, and have taught courses in Cancer, so it is really bizarre to feel like I am now my own "case study".
I will indeed keep you all posted as more is uncovered and as my case progresses. I am trying to stay hopeful. I am grateful to have found this community!
