Jardiance for stage 3a CKD or Acute Kidney Injury (AKI)
A month after a nasty case of Norovirus on 12/23 my creatinine levels ramped up and my eGFR went from 77 to 35. Other than being 71 yo, I have no risk factors for kidney disease, not family history, BP, pre-diabetic, or diet. I have elevated proteins in my urine now so started on 10 mg Jardiance sglt2 inhibitor (I don't tolerate ACE II) to protect my cardiovascular system and slow/stop progression of CKD or AKI. I will find out side effects, but was wondering if any of you have had irksome ones. My glucose has never been elevated so I'll probably carry hard candies now because Jardiance decreases blood glucose. I will have to drink more water and start chugging cranberry juice (preventative for UTI, lessens the ability of bacteria to adhere to urethral walls). Being a lab rat is getting a bit tiring.

About the Cranberry juice, I read this a few weeks ago to keep in mind: Most cranberry juices contain added sugar, which can negatively impact kidney health, so choosing unsweetened or low-sugar options is important.
I found a cranberry juice that was only 5 grams of carbohydrate per cup (8 fluid ounces). It said on the bottle “no added sugars” and had a big endorsement by JDF (juvenile diabetes foundation) on the bottle. I think the brand was Old Orchard and it as labeled “Cranberry Light”.
Keep in mind when you are looking for 'sugar free' that you are staying away from the 'sugar free substitutes' that can cause harm to your health in other ways too. I suffer from Migraines and too as well as having Stage 3a Kidney disease. I have had to watch out for these little 'sugar free substitutes' for many years because they can cause my Migraines to flare. Aspartame not only can cause Migraines, but it is know to cause kidney damage too. I know some sugar free candies has this sugar substitute in them. Not sure about the cranberry juices, but I thought I might put this note out here just as a caution. I can't even use the sugar substitute Stevia it does the same.
I use 100% juice cranberry, brand 365 is just water and cranberry. Because I have a few autoimmune diseases (celiac, colitis...), my diet has been low sugar (no artificial) and salt, and mostly FODMAP and gluten free for years.
Ibuprofen is the worst medication to take for CKD. Nothing with Nsaids should be taken with kidney disease. When I was first diagnosed with kidney stones I was told to take 800mg of ibuprofen every 6 hrs. That is what helped my CKD escalate. That was in 1997. I am now Stage 4 Severe but am told that everything is fine as long as I stay away from salt, high potassium foods and ibuprofen. My eGFR2 value is stable at 29. BUN 44
Creatinine 1.80 Uric Acid 8.8. I had an excruciating gout attack waking me up from a deep sleep last week. It lasted 2 days. So it seems something is getting worse. But none of my doctors seem alarmed.
You also have to be careful with diuretics as this can cause potassium and uric acid to go wacky. But from what I have been told and researched, Jardiance and Farxiga are great for both T2 diabetes and kidney disease. I am on Farxiga and it will makes you pee like a racehorse. So make sure you drink plenty of water. I have to wear diapers 24/7 so it is somewhat of a pain but if it works it is definitely worth it.
I also have grandparents and great-grandparents who died of kidney disease.
I can feel your pain. I am on dialysis 3 times a week. Months before that I was taking 400 mg. Allopurinol. Dr. told me it would be better for me to make it 100mg. Ok for three months that worked, then blood work came back with a uric acid level of 8.
Claimed he didn't know why. So I got a Dr. to treat me that knows Gout. I now take 200mg. But it was a long road back.
@tntwo99 I also have gout. I was on allopurinol 100 mg once a day, for over 10 years. Six months ago we decreased it to three times a week. My uric acid check two weeks ago show it at 6.9, just below the upper limit for my demographics.
One thing for gout, while also dealing with kidney disease, is to be mindful of shellfish, pork, and purines [like mushrooms]. Also, processed meats like deli meats, but that is something you need to watch on a renal diet, too.

This discussion is getting confusing. I keep scrolling back to the title which is about Jardiance, CKD and AKI. I know Jardiance as a diabetes medication, but this is the Kidney & Bladder group. Granted, diabetes is one of the common causes of kidney problems so I get that. But aren’t CKD and AKI quite different (related but different)? Then we morphed to talking about cranberry juice (myself included), then sugar substitutes, then ibuprofen, then gout …..and so where are we now? I think @tntwo99 came close to the original post with “Jardiance and Farxiga are great for both T2 diabetes and kidney disease” based on some research having been done. This discussion is worth at least 6 or 7 topics and is really interesting!
As the author, I was interested in unusual or irksome side effects from Jardiance.
I don't have diabetes, but following norovirus on 12/23, I have stage 3a CKD and/or AKI. The nephrologist prescribed it not for glucose but the side effect of decreasing my creatinine to protect my heart (which is currently in good shape). I have no symptoms from CKD or AKI yet. After 7 days at 10 mg/day I have no apparent side effects from Jardiance, including that I don't pee more than I normally do. I'm a retired research scientist, so if people are interested in SLGP2 meds, I could post some links. Regarding @tntwo99 's comment on diuretic effects, pelvic PT that includes Kegel exercises help to decrease urinary incontinence.
