Is it normal for free light chains to fluctuate?

Posted by nick86 @nick86, Oct 25, 2023

Hi all, I hope you're all coping okay with whatever life is throwing at you at the moment.
I have IGG lambda MGUS. My M-Spike has remained stable at around 12 g/l (1.2g /dl) since my last blood test 3 months ago. My Lambda light chains have risen from 22.17 to 30.10 in that time. However my free light chain ratio is still within normal range. My Kappa light chains haven't moved. Is it normal for light chains to do this and what happens if it keeps rising?
Best wishes to all.

I am new here, and salute all my fellow sufferers with a sincere wish for your complete cures and ongoing perfect health. I JUST CAME BACK FROM MY 2ND BONE MARROW BIOPSY. My doc sent me home with Dexamethasone to take for 4 days at 10 4MG tabs per day; I assume the reason for this was my LAMBDA LIGHT CHAIN, FREE, SER/PLAS test came back at 7507 mg/l. Being new to blood cancer, although I also have prostate cancer and am recovering from sarcoma of the sinus (ops and radiation), this seems somewhat too high for my taste. If divine help is in order, I humbly ask for prayers from my compatriot Christian believers, as I will also pray for you.

I also have IgG lambda with a M spike of 1.5. Was diagnosed in 2002 and my M spike has slowly risen from .4 in 2004. FLC were not available then at least to patients like me. That said, my flc have wandered all “over the place” from measure to measure. My ratio is abnormal. Hydration at the time of the blood draw makes a big difference.
Also I have had four major joint replacement surgeries since 2015. Each surgery resulted in my flc increasing.
My two most significant increases were in 2021 and 2023. In 2021 my hip basically collapsed with the femoral head breaking off. The very painful period of time in the months before surgery saw the greatest increase. (It was Covid time and hip replacement surgeries were considered elective!?!?) Surgery was finally done under regional and the flc subsided. So it was clearly inflammation. In 2023 when I had the shoulder done, it was under general anesthesia. M flcs were 20 at the time. Again the flcs escalated! They have come back down but are still very abnormal (27.1).
I feel good. Walk 4 miles a day and swim for an hour . Am wondering if these are polyclonal rather than monoclonal. My hematologist believes they are monoclonal.
