Is anyone dealing with postprandial hypotension?

Posted by anet @anet, Jul 5, 2023

My husband’s blood pressure drops after meals sometimes. He had never passed out, but it is scary, He just feels weak. On one occasion his BP was 75/60. He has CKD and also had aortic aneurysm surgery a year ago. His blood pressure fluctuates up and down quite often, but then will be normal for a week at at a time. His kidney doctor has him taking 2.5 mg of lisinopril when his BP is over 130. Just wondering how others treat this condition. We are seeing his cardiologist this week.

Hi @anet, There is another discussion started by @bumble81 who mentioned having postprandial hypotension here - Digestive issues, weird sensations: https://connect.mayoclinic.org/discussion/digestive-issues-weird-sensations/. @stefspad, @elizabej and @bonnieb81 have mentioned postprandial hypotension in other discussions and may have some thoughts or experience to share with you.

You didn't mention your husband has any abdominal pain after eating but I thought this article sounds similar to what you describe is happening after your husband eats a meal.
""Can low blood pressure cause abdominal pain? — If you feel abdominal pain and dizziness after eating, it may be because your blood pressure hasn't stabilized. This sudden drop in blood pressure after a meal is called postprandial hypotension. Normally, when you eat, blood flow increases to your stomach and small intestine." — What's Causing My Abdominal Pain and Dizziness? – Healthline: https://www.healthline.com/health/abdominal-pain-and-dizziness.

Hoping his cardiologist can find some answers at your husband's appointment this week. Are you able to provide an update after his appointment?

He does not have any pain. Just the drop in BP and sometimes gets a little wobbly. The cardiologist decreased his BP medication. He has cardiovascular disease and a slow heart rate. After eating his body directs blood to his digestive system. That causes the drop in blood pressure for a few hours. He has a low heart rate anyway and his heart has to try to speed up and try to get the BP back up. Doctor is doing more tests and says he may need a pacemaker.

Hi anet. Do you have any more information on this? Because two months ago I had heart surgery for an aortic aneurysm also. Since then, every time I eat breakfast, my blood pressure drops and I get lightheaded and wobbly. I brought this up to my primary care physician, but have not heard back regarding it.
I have no stomach pain and I take no medication’s (no blood thinners, high blood pressure meds, etc.) because I thought they may be causing it.
Do you have any more information regarding this issue?

If you have a home BP monitor start documenting your readings before
and after breakfast. This could help your doctor determine if you need
to have further testing or medication.
Fall awareness is very important and an emergency plan is a good idea.
Smaller meals with more frequency might work. Drinking 8 ounces of cold
liquid after the meal may raise your pressure enough to navigate safely.
I had to delete meds and drink more fluids. Now I no longer avoid salty foods
but still required a prescription to raise my BP.

I do have a home BP Monitor and watch it closely. It’s always high right before I eat and then goes below 100/60 after I eat. This morning I went from 6 AM to 9 AM and checked it multiple times and right before I ate and it was 150/100 the whole time. Ate breakfast and 15 minutes later it was 90/55.
I get lightheaded and have to be careful when I stand up.
I’ve done the same, stopped all meds and drink a lot of water. I’ve also stopped avoiding salty food as it helps get my blood pressure up a little bit.
Maybe I better discuss this with my primary physician.
