Incontinence and coffee

Posted by edinmaryland @edinmaryland, Jun 14, 2025

71 y.o had RARP in January, nagging incontinence since however I can see improvement, less leakage, better in the mornings, dry at night Still, I imagine I have at least several months to go if I am lucky.

I am taking Gemtessa and have tried to avoid bladder irritants. It has been five months since I have had coffee and I miss it.It really is my only vice. I understand that both the caffeine and acid can be bladder irritants and I certainly do not want anything to make me go backwards. However, I was thinking of trying a half cup to see what happens (low acid, maybe caffeine free for starters. has anyone else had experience with re-starting coffee under these circumstances?? thanks much

Here's the response I get from my Oncologist and Urologist. "Wow, your the first
patient we've had with these issues". LOL I stopped even bringing it up. I've had more
luck researching it myself. Good luck with your situation. I'll reach out if I find out more.
Thanks!

I am 73, also had RARP in January and am on Gemtessa. I restarted coffee one cup in am with only minor increase in urgency for an hour or so. I am an MD and know of no reason that this effect would be anything other than temporary and so I think you could safely give it a try.

Thanks I am now carefully drinking a small cup of coffee probably five days a week. I usually drink a full glass of water before and after to dilate it
I am getting ready to discontinue the gemtessa as the urge incontinence is not as bad as it was. Stress incontinence is better but by no means resolved. Thanks for the d feedback I am at 15 months and hopeful progress can continue however I may next be a candidate for salvage I hope that does not start things all over

Pound 16 ounces of water first thing in the morning and eat something then drink your coffee and see what happens. We are all different and self-exploration is an important part of getting back to our normal or new normal life.

I’m able to drink 1 cup in the morning ok with nothing but creamer. I noticed if a use an artificial sweetener it seems like I leak more. I try to drink it every other day and skip some days. And I do love my coffee. I’ve had the salvage radiation

@jrs619 thanks to both you two. Did the incontinence come back after the salvage?

Try caffeine-free "decaf" coffee and tea. It helped me. Besides the caffeine and the acid, the other fact is you're likely just taking in too much liquid, and per unit time. I went to 7 out of 8 Pelvic Floor Physical Therapy sessions, and one thing I was told, is to not drink more than 8 ounces of anything at any given time. We have to find a balance of drinking enough to satiate us, but too much to cause leakage. And, we have to drink enough to keep our bladder from shrinking, but also not too much to keep us leaking. Frustrating...damned if we do, damned if we don't. Good luck to you.

I did find some special coffee on Amazon that was caffeine free and low acid. I am drinking regular coffee now just a small cup maybe five days a week. i think it is going OK.
I was not aware of the 8 ounce rule. My PT kept telling me to drink more and more there was some rule about 80% of my body weight in ounces which was close to a gallon. I never drank that much water before the surgery but I pu8shed it and tried to drink more. Now I tend to drink more than I would have before but not a gallon a day. Sometimes in the morning I drink probably close to 16 ounces counting about 6 ounces for coffee.

I am probably not stretching my bladder enough as I almost always get up in the middle of the night and urinate.
I am still incontinent but it is better than it was. Climbing a ladder. picking up grandkids, sometimes lifting something heavy causes leakage. I am pretty close to normal in the mornings.
I religiously do two sets of Kegels every day ( five warm up, five progressive, five long holds (10-15 seconds) then 4 sets of short ones

any feedback?>

On a different subject for those that had to have salvage, did this cause a set back in incontinence?
