i have only had approx. 4.5 hours of sleep in almost 5 days

As i stated in my question, I haven't slept pretty much at all in the last 5 days. below is approx. sleep and dates.
25th July, been awake since 10am however this nite i only get about an hour of sleep between approx. 11pm and midenite. Woke just after 12 am on the 26th.
26th July, got approx. 3.5 hours sleep between 5pm and 8:30pm. did not sleep after...
27th July Nil, still awake.
28th July Nil, still awake.
Currently 29th of July and 5:06pm and still no sleep.
I suffer from:
extreme obstructive sleep apneoa (so those few hours...
were not real sleep)
horrible anxiety / Depression
social anxiety
OCD and ruminating (reapeat the same things, and constently checking ive done things correct, second guessing my choices, i think thats what is getting me through writing this in this state and it be understandable)
PTSD
some substance abuse (Weed n Beer) used to be much much much much worse and currently treated with gov services).
im scared, should i go to hospital, im also scared because im starting to be able to function again (havent for sometime). I also dont think im getting any sleep soon...
I don't know what to do, im not sure if its cause my thinking is impared or im frightened...

You have enough things ganging up on you that you probably need to see a therapist who can help you to get off the ruminations, worry, anxiety, etc. You might need, for a few days only, some sleep aids, but those SHOULD come via prescription and after consulting a competent authority such as a physician, ENT (ear, nose, and throat specialist, formally 'otolaryngologist'). I think you can bust this cluster up quite a bit, but it might be tackling it on several fronts at the same time. Be patient with your helpers, with their prescriptions, and with yourself. One of your best tools will be optimism, and the other will be accepting that there is some help out there.
I went 5 days without sleep after my ex left me. It was horrible. All kinds of weird symptoms. I totally relate to the anxiety you are experiencing. Whole heartedly agree with @gloaming’s advice. If you have a pcp I would call the office and ask to speak with the nurse

Are you being treated for your sleep apnea and using a cpap? Do you take any antidepressants and/or antipsychotics to help with your anxiety, OCD, PTSD and are you getting therapy/counseling? You may need to check your sleep hygiene (environment, caffeine, screen time, etc.) to help you prepare for sleep and keep a structured bedtime routine/time. Do you take any antidepressants bath before bed and take melatonin or Calm magnesium to help you relax before bedtime? Have you tried herbal teas, sound machines, dark curtains for bedroom, etc.?

Going 5 days without sleep would do that, regardless if you had sleep apnea or not, the body alas needs rest, rather hard to do when you cannot sleep.
My average sleep time is 5 hours a day, has been for the past 10 years, unless knocked out sleep assisting meds do nothing for me.
I may go to bed at 10pm, body vibrates for approx 3 hours keeping me awake, then I sleep for 5 hours, was scary at first but have learned to live with full body vibrations for past 40+ years.
The biggest reason people fail to sleep is anxiety or worry, it prevents the mind from switching off.
I find a simple solution to this and no it doesn't involve pumping your body full of drugs, all it involves is a set of headphones and white noise playing in the background, be that rain or wind turbines the latter I find more consistent than rain.
Calm the mind and sleep will follow
You do need help. First of all, sleep. I used to watch the sun come up without it. You can lose your marbles if that goes on too long. I take Ambien which makes me sleep. Without it I would be lost. Lorazepam 1 mg is helpful if I'm too anxious for the Ambien to work alone. You might consider going to the ER sometime during an episode. I got more help there than from my GP doctor. I tried a CBD and THC oral concentrate combination but I didn't like the supposed "high". It felt like the stroke I had last year. It didn't make me feel good at all. Maybe CBD alone might help? Stay off sugar too! It's poison for people like us.
I have empathy for you with the sleep and ex issue. Lack of sleep for me is the biggest trigger for my seizures. Curious, did you have any seizures during your 5 days?
I did! I was on wellbutrin at the time which lowers seizure threshold. So they took me off.
Please find someway to overcome this sleep matter. Having lack of sleep seizures are bad, but if left unchecked, lack of sleep in general destroys a person's body. My best for you.
