How to send my chart along with MRI and CT Scans

How is the best way to send my chart info along with MRI & CT Scans from an ER visit to Mayo for a second opinion?

@kaylame Welcome to Connect. What type of specialist are you looking for? Since your post is in the headache group, is that your concern? What other symptoms do you have? It really helps to focus on a particular department when you apply to Mayo. They will set up a patient account and tell you how to submit records and imaging. When that is received, it will be copied into the account for you. After all of that, doctors will evaluate and determine if they can help. Mayo gets a lot more requests than they can handle. You may also have your doctors help you request an appointment. What are the symptoms that are most concerning?

You would want to complete the medical release information Mayo Clinic requires and indicate where you need your medical records sent. You can call the location you went to or look online for that facility’s form/process. Below are some links I found.

kaylame, I'd request paper and CD copIES from the ER, because I'd want copy for myself and because it's faster. Mayo will copy the reports and return the CDs if requested. I always FedEx or priority mail the scans. To speed the process. Call the Mayo department you wish to be seen in and get the mailing address.
It's easier to let them gather your records, but I'm always in a hurry.

As mentioned by mentor, you should set up a patient account first. I originally called into Mayo (specific Dept) and they instructed me to do that and then have my records sent to Mayo. I set up the patient account and I instructed my local provider to send all my records to Mayo. Their Records Dept knew where to send at Mayo(fax number etc.). I called local provider back to be sure they sent the documents and checked with Mayo to know when they were received. I was fortunate because I could self-refer to Mayo.
You can see your ER records, and Dr Notes on provider Internet site. Read them to see if they are accurate.
