How can I find out which type of cyst I have?

Posted by bosnia @bosnia, Nov 22, 2023

Hello everyone, thank you for accepting me. On 6th December 2021, I had an ultrasound, and the doctor found a cyst at the body of pancreas. Dimension 3.9 cm x 3.5 cm. I did MRI four times and twice a biopsy, but I didn't find out which type of cyst I have.
I haven't got pancreatitis.

After first biopsy size was 3 cm x 2,4 cm, but in August 2023 it grew to 5.18 cm x 4,65 cm.😞 I did MRI-MRCP two weeks ago and size was 2x2,4cm. Also, I did EUS-FNA it is benig now, but i still don't know which type is.

CA 19-9 was 20.67, now is 28. CEA 103, Amylaze 169.

After the increase in size, doctors suggested resection. I went for a second opinion and they suggested monitoring. I am so confused now.

Sometimes, I have pain in my back and stomach. Have we any tests that can reliably detect the type of cyst maybe KRAS mutation etc.?

Kind regards,
SP

Hello @bosnia and welcome to Mayo Connect. I can certainly understand your concern with the pancreatic cysts. I, too, have pancreatic cysts, that are monitored regularly. It is good that your FNA showed that the cyst was benign.

Unfortunately, I can't answer your question about the type of cyst. As you may know, on Mayo Connect we are not medical professionals who can give a diagnosis or other medical information. We are all patients, like yourself, who seek to support and encourage each other.

As you have some concerns about the change in size of the cysts and also about the Amylase level, I would encourage you to get a second opinion. Are you nearby a Mayo facility or a university medical center?

Second opinions are good ways to reduce your anxiety and concerns. Will you post again and let me know how you are doing?

Thank you for answering me. I know we all are patients, but i am interested does anyone else has same problem or everyone know which type of cyst they have.
Because, I read that some other patients know which type of cyst is after MRI MRCP. I still don't know after two EUS FNA. 😟
I am so far from Mayo Clinic, because I live in Europe.
I have opinion from the best doctors in my county that i need a surgery. And two opinion from other countrys, that i need a monitoring. I travelled to another country to do MRI MRCP and EUS FNA in clinic which has the best results in cure a pancreas cancer.
I am satisfied, but still investigeting how find out which type is.

Hello @bosnia,

I found some information on Mayo Clinic's website about the two different types of pancreatic cysts. Here is the link to that information:
https://www.mayoclinic.org/diseases-conditions/pancreatic-cysts/symptoms-causes/syc-20375993

Have you asked any of your doctors who have done the FNA or MRCP this question? I'm wondering if they would tell you, or if it would be on the biopsy report. Did you get a biopsy report for your own records?

Thanks Teresa for information. Cytological findings show that it has a cylindrical epithelium. This would support the fact that it is not a pseudocyst, because a pseudocyst has a fibrous capsule. CEA 103, Amylase 169. Conclusion: cystic change. I asked the doctors, but they don't have concrete answers. It seems to me that the parameters do not fit into any known in the literature and that is why they avoid specifying the type.

You could be right about the reason for not specifying the type, @bosnia. I wish you all the best as you continue to follow up on these cysts.
