Just a quick note to say how helpful, your post was for me, in particular, your honesty and hopefulness.
I’ve got BPH and low grade (3+3) prostate cancer. Had PAE done a year ago which has helped the BPH and allowed me to quit taking flowmax (which I disliked because of lack of build/explode/release feeling at orgasm and stop finasteride (which had knocked down libido and seemed to contribute to some ED challenges).
My wife wants to stop with sexual activity for now - I think a combination of my ED and her changes with age - so cautiously stepping back into some 60-year old version of masturbation. It’s different now approaching my body with open curiosity in its new responses.

Hello- sorry to hear about your other urinary issues. You are very fortunate that you don’t get up to pee at night- still 2-3 times per night for me!
Everything you said is what I considered too. I’ve heard Aquablation might have some better outcomes sexually. That might be worth comparing Holep to.
I had also a very large amount of urine I was not emptying, thus my infections wouldn’t go away.
For me, my peace of mind that I am now urinating like a teenager, completely emptying my bladder and no more infections along with knowing my Holep has excellent data showing that it is a long-term solution. I also no longer take any prostate medications. To say I was terrified of the surgery was an understatement. As long as you have confidence in your doctor (and has done many of whatever procedure you select) will ease your concerns. But, I made it through the other side now, so I only try to be positive and hope for some improvement in the future.
Considering that discussing the quality of one’s orgasms is about as personal as you can get! I too am not pleased with how my orgasms are right now. I have read that with all the nerves involved in the groin/genitals, it does take some time for everything to heal up as before surgery.
Considering your other issues mentioned, something has to be done before things get any worse. When the doctor mentioned possible kidney damage from not emptying my bladder, I had to make a choice. There is no ‘perfect’ option, unfortunately.
It is great that you are being open about all of this. So many men refuse to do anything that relates to their penises, simply because of fear or lack of knowledge. All the best to you and whatever decision you make!

Hello- glad to know this was helpful. Some of the men I read here have similar issues and this is such a personal, private part of our lives that men just don’t talk about.
We all seem to agree that our orgasms are very important to us. My doctor knows how important this concern was for me!
Life can be so hard sometimes. Masterbation is something that can be wonderful. I know for me, I now actively have intercouse but also plenty of masturbation. My wife understands that my body has changed and that I need time to relax and explore what feels good to me, again. It’s not like I am a teenager again, but I no longer have any embarrassment, shame or guilt- if I want to masterbate, I do! And my wife appreciates that she doesn’t always have to be involved, especially when my sex drive ramps up.
