Help - One sided pulsatile tinnitus and neck pain same side

Both started simultaneously last summer (2025). I had an ear infection at that time too. While the infection cleared with an antibiotic, the neck pain and one sided pulsatile tinnitus (same side) have continued daily - very hard to sleep. I am getting physio for my neck but it isn't helping the pulsating one-sided tinnitus. A spine surgeon looked at the brain, neck and mastoid imagery from my ENT and Neurology doctors (I had CTAs to rule out any blood flow or blood clots), and the spine surgeon said the cerival C4 and C5 were eroded and compressing against each other. Has anyone else had this dual problem of neck pain and bad tinnitus starting at the same time, and what was the outcome (if any)? Mayo Clinic acknowledges Cervical Tinnitus as a real thing - my spine doctor (a surgeon) disagrees there is any connection. But he recommended a cervical traction collar to be worn once a day for a week and see if the neck feels better. Anyone have any related info?

My concern about neck traction is the possibility the cause is compression of the left vertebral artery, which could seriously make things worse. Since the left neck pain started simultaneously with the left pulsatile tinnitus, I'm hoping for a second opinion on my CTA/V of the neck.

Hi, was wondering how you're getting on? I've left sided neck and shoulder pain which started after lying in awkward position plus pulsatile tinnitus which started at same time, it's really getting me down.
I've read online (dreadful I know!) that neck strain and pulled neck muscles can cause pulsatile tinnitus and hoping once the strained muscles ease the tinnitus will go too.
Going to contact Dr tomorrow as had a week now and should get checked out.

Hi! New to the forum. So glad I found you all! Started with major neck pain last spring. It was off and on for awhile but now a little more frequently. Then this fall, I woke up one morning with my heartbeat pounding inside my head. Went to the ENT, and at first they told me I had sudden sever hearing loss. Then they did an MRI and nothing was amiss. In the meantime I was put on prednisone, which took away the pulsatile tinnitus for about two months. So since he couldn't find anything he directed me to a neurologist who did an MRI of the neck, an MRA, and a CT scan of the head and neck. The neurologist found nothing vascular or arterial. The doctor that read my scans said I had normal for my age wear and tear...some arthritis and a bone spur and a slightly bulging disc. I don't see the neurologist until April. At the end of January, however, the pulsatile tinnitus reappeared, and about a week later, went away, this time on it's own. Then just yesterday, a month later, it returned. Before it came back yesterday, I started with this very nervy feeling headache and sometimes it's worse than others. Before last week I had no headache with the tinnitus or neck pain. My neck pain is both sides of my vertebrae but my tinnitus is just on the left side. Headaches are on the top and back of my head, sometimes they feel like they are behind my eyes. But I'm literally at my wits end. My question for you guys is, how does the pulsatile tinnitus make you feel physically? I could feel it coming on a few days before it actually re-appeared. I'm not sure how to describe it. Is anyone else's pain and tinnitus off and on or is yours just constant? I almost feel sick when it appears because it makes me so anxious. It's so disruptive to my every day living. I do have to say that I'm 50 and definitely in per-menopause. So not sure if this is all related to hormonal fluctuations or what. But it's the pits.

I have had one sided pulsatile tinnitus- left side over four years now. I saw an ENT surgeon who ordered a CTA which I was told was negative. No clots, aneurysms or tumors. I then saw my cardiologist who ordered a Doppler to check my neck for a vascular issue. That was negative as well. I even had my husbands neurosurgeon look at my CTA, he too said not to worry about it. I have had neck issues for years with MRI showing degenerative disc disease but not sure which ones were bad, but it’s gradually got worse with age. My neck cracks, hurts, and sounds like something is grinding in there and is worse some days than others. I too, have had PT which seemed to help when I was going. I haven’t for a year now and can tell. When they stretched my neck that seemed to alleviate the pain and I had less tinnitus. I do have high BP but is controlled with meds. My doc said it’s not that. So I’m not sure what’s left. Has to be a pinched nerve on an artery or something. I’m not clinical so I can only guess. You’re not crazy though, this is very real.
