I'm so sorry you are having so many issues! That must be terribly hard to deal with. What does your doctor say about everything that is bothering you? I can suggest a few things such as putting Metholatum in your nose to help with the blockage. I do that and it does help me to breathe better. Also take a hot shower and then put cream all over your body before bed. This is very relaxing and helps you to sleep. It is very good for your skin and will help with the stiffness in your muscles and hip. A nice soft flannel nightgown or pair of flannel pajamas are very relaxing and help you sleep good. I love my flannel nightgowns! They are so comfortable to sleep in!

For your other issues, I would suggest that you take your problems to God. He made you and he can fix you! Just talk to him like he's your best friend and he is! I will say a prayer for you also.
I wish you the best.

Aww sweetie, that is a lot.
It looks like each need to be dealt with independently.

I would suggests warm bath followed by getting stretching of the areas that are stiff. If a bath isnt possible warm the muscles up with heating pads, heating blanket, hot water bottles, and such, then gently stretch them

Tooth decay must be examined by a dentist. If it is reoccurring, maybe there is something missing from yoir diet.

Ruminating thoughts can be replaced by Socrates questions that help you make sense of them. You can Google Socrates questions to get a list, then pick the ones that will work best for you.

Our bodies are very sensitive to changes and love predictability. If you can get yourself into a relaxing routine you may be able to help yoir nervous system to relax.

If you are in the US you may be eligible for a free workshop for chronic pain and chronic illness. https://med.stanford.edu/pain/patients/patient-resources/free-lectures-and-support-groups.html. If this is not your area you can contact them and they may be able to help.
