Has Anyone Had Dental Work (implant, extraction) and Not Gotten ONJ??

Having dental work -- specifically, a tooth extraction or an implant -- is supposed to be a risk factor for ONJ if you are on a bisphosphonate or other antiresorptive medication. I was wondering if anyone in this forum has had this kind of dental work but NOT gotten ONJ. My brother has been on Prolia for several years and had extensive dental procedures to extract teeth and put in dentures, but has not had ONJ. As you might guess, I am posing this question because of fears of taking a bisphosphonate after being on an anabolic agent (Tymlos). That is required to prevent loss of the newly formed bone.

I had an infected implant removed a few weeks before my next Prolia shot was due. My oral surgeon said I couldn’t go back on Prolia for 4 months and I was petrified. I continued to work out with a specialist PT who is knowledgeable about osteoporosis. My Rheumatologist told me at my age, 79 with lots of arthritis is impossible to get an accurate bone density on my spine. My forearm was 3.4 and my hips were osteopenia. I was petrified since she said going on Tymlos for the first few months would have a rebound effect and probably not be effective. Last Tuesday I took my Prolia shot. It’s not a magic bullet so I know I still have to be careful and not work out too hard. I hope I’ll be one of the success stories. I’m still a bit nervous because of my age. You must find knowledgeable people that know what they’re doing. I was already on the pills when I was younger so it wasn’t an option. Fingers crossed I’ll be ok.

Sometimes I think I read too much. I was looking up if restarting Prolia can put you at risk for spinal fractures and it said it also had a rebound effect and I need to be careful for several months. I’m beginning to dislike Prolia even more. I’m working out and doing everything they tell me. I pray I’ll be ok. 🙏🏻

I'm on Tymlos as we speak and also in the middle of getting an implant. My oral surgeon was not alarmed because Tymlos is a parathyroid hormone analog and doesn't increase the risk of ONJ.

Yes, Tymlos is not a problem. It's those bisphosphanates that can wreak havoc!!

I had two extractions and one implant while on Prolia and did not have ONJ.

Thank you for relaying your experience! Good to hear!! I've heard from several people on Prolia who had no issues with extractions/ONJ. I wonder if the incidence of ONJ is different for Prolia than for bisphosphanates. Hmmm...I'll have to do some digging around the internet.

I think it’s very rare and some people get lucky. My dentist is very strict and I did follow his instructions and went off Prolia for 4 months. I went back on it 4 weeks ago. I’ve been working out and increasing my weights slightly every week. I pray I’ll be one of the lucky ones and not get a spinal fracture. I don’t think my numbers are as bad as some. I do a lot of core exercises and hope for the best.

Tymlos is fine. It does not give you ONJ.

Yes, definitely. Tymlos is safe. The problem is the bisphosphanates. And there are warnings about Prolia, but people posting here have not had problems.
