@jessgarduno There are many factors when looking at test results. Together they are interpreted by your doctor, based on what your medical history is, medications you might be taking, lifestyle, diet. Even different labs to process your blood samples, can make a difference.

I recently found out the GFR calculation changed. I was always monitored by CKD MDRD type GFR calculation. All my historic records for GFR monitoring were erased in my records and they started new this Sept. with the CKD EPI creatinine equation.

"On September 24, the Task Force’s Final Report recommended the use of the new eGFR 2021 CKD EPI creatinine equation that estimates kidney function without using race. The Task Force also recommended increased use of cystatin C combined with creatinine for a reliable way to check kidney function."

My GFR is relatively the same, improved a bit from 25 to 30. Theoretically, kicked me back into 3rd stage rather than 4th stage, according to numbers. But more important is stability over time. The numbers go up and down, as they have for me over 8 years but remain somewhat stable. My BUN shows constant dehydration, but that's part of my disease. I'm always dehydrated, no matter how much water I drink.

I used to get upset when my numbers went down and confused when they went back up. There are many factors why that happens. Time of day of the blood draws, exercise the day before, how much water you drink, etc.

You can get blood draws every 3-6 months and watch the pattern. It's really not a concern unless something drastic shows up. A huge drop in GFR or increase in creatinine. I've learned all this over time and through my appointment at Mayo with a Nephrologist. Blood work is basically monitoring, so there is no reason to panic about number changes. As I stated in a previous post a while ago, my Neph at Mayo said many people live a long time on very low GFR numbers. There are so many factors, what kind of disease you have, how quickly changes take place, etc. It's important to get the right diagnosis. I was misdiagnosed by my local Nephrologist. Since then that disease has been taken off my records (AKPKD). I am continuing my relationship with the Mayo Clinic although there is no treatment for my "real" disease. Much of this has to do with how you feel. That's what you should be monitoring. I have no symptoms.

My Cystatin C went up from 50 to 54 in the same time frame that my eGFR (creatinine based) went down again to 41 (36 when not hydrated). So Cystatin C GFR seems important to do. We were making clinical decisions based on the idea that my kidney function had worsened but they now think the creatinine-based GFR is lower from weight loss/muscle mass loss.

Hello' so I question. I go in today for my blood work fellow up and looking over my labes and seeing if anything seems out of the ordinary. The urine creatinine was 216 in 2021 now its 74 in 2022. They haven't done any new test as far as urine samples for the creatinine levels since then. But it's not flagged on the blood work either. So I happen to see it. Among other changes in the blood and health I wanted make sure I have all the information I needed when I go in today to discuss my findings and they just don't brush it off. Thanks for replying

@jessgarduno30 There are several factors to consider in your blood tests. Hydration level, exercise timing, diet, medications, overall health [other contributing conditions, for example], stress, even the lab itself performing the test.

What are your concerns?

Well... I was or still am pre - Diabetic. Cholesterol is never been good. My tsh levels are sometimes right but then out of wack which I can handle and know all about. Thyroid doesnt work since birth. glucose levels went down alot being on Metformin. I am not over weight but not under. I have bipolar 2 so all that comes with that. But I can't figure out why my kidney function would fluctuate up and down but not be flaged and then being more vitamin d
Deficiency which I can see part of that but I have always been low but now really low. Blood in urine but no protein.. Then I had a chronic Urticaria which was tested and said it was autoimmune related. Immunoglobulin was the only thing down it was 4 just below. But never tested and more they said just take allergy meds. So I am concerned what health problems are really going on and why aren't they being flagged.

I reached every thing and crossed researched everything is in range but those few things
