Does surgery work for Laryngopharyngeal Reflux (LPR)?

Since December 13, 2016. My symptoms started with a soar throat, soar inflamed taste buds on the tip of my tongue and inflamed salivary glands on my soft palate. My tongue, throat and soft palate had a painful burning sensation that was on and off. My General practitioner referred me to an Ear Nose throat specialist who referred me to an oral surgeon who referred me to an oral pathologist for burning mouth syndrome. After wasting 3 months of running around with these Dr's and being tested for oral thrush, hsv tests, vitamin deficiency tests, auto immune deficiency tests, cultural bacteria tests all which came back normal. I decided to go for a second opinion with another E.N.T. specialist who performed a camera scope and diagnosed me with acid in my throat. I was put on Dexilant and noticed the pain dissipate almost immediately only problem was I couldn't tolerate the medication. I was then put on Tecta and referred to a G. I. My general Practitioner did some blood work for h pylori and I tested positive. I started to develop tinitus in my left ear. I had a gastroscopy where they found a 2 cm Hiatel Hernial and my lower sphincter was weak. Thats when I was told I have a mild case of gerd, but I don't suffer from any heart burn. I was put on triple dose of antibiotics for h pylori and continued with the Tecta, domperadone and sucrolafate. I'm getting a little relief from the PPI and meds but I still suffer from bitter reflux taste and burning pain in my throat, soft Palate, the tip of my tongue and dry mouth that comes and goes. I'm in the process of having a 24 hr ph level test done. The G. I. I was seeing says my esophagus biopsy tested clean and he doesn't believe what is happening in my oral cavity is related to my gut. He stated in order for acid to travel to the throat it has to travel through the esophagus and the esophagus tested clean. Thus he can't help me anymore. I'm so confused my ENT says I have acid in my throat and the G.I. doesn't agree.

I"m sorry you're going through all that! Is that one of those little camera's that you swallow with the monitor? That would seem like the easiest way for them to have a direct up close look inside your esophagus. I've decided to switch to Nexium because I've heard a lot of anecdotal evidence that it works much better than the omeprazole. Good luck with your test results! It must be very frustrating for you.

The test I had was called an EG Bravo, they put the camera or whatever it is to monitor acid level. It gets inserted in esophagus during an endoscopy. Mine showed some elevated acid but not off the charts as some have. I had the Manometry test next. Its not a fun test in the least but it is a good test. It showed that my esophagus is not working properly. My Dr. said it shows that my esophagus is only pushing food down like it should 20% of the time. So I have a lot of throat clearing, bloating, have to eat small amounts of food at a time, watch what I eat for GERD. Dr. put a diagnosis of "weakened parastalsis, possibly Accolasia. He said there is no surgery for this. He is referring me to a gastroenterologist who deals with this specific diagnosis. I started drinking Lily of the Valley Aloe Vera juice and I believe it is helping . I will continue the juice no matter what. I watched a video of info on the juice on youtube and everyone could benefit from drinking it.

Yes the Omeprazole helped with the reflux.
I am now taking Rabeprazole 20 mg once a day. That does help with the reflux and I have no side effects that are bothersome.

Hi. My problem started about a month ago when I started chugging down a glass of water and suddenly I had a spasm in the area of my pharynx. It scared me, and took several minutes to abate, but no other symptoms. Soon, I found that I could no longer drink orange juice without the same problem, and then ice cream gave me very thick saliva that was difficult to spit out as well as the tightness in my throat. I had had problems with normal heartburn in the past, but a few days of ranitidine (Zantac) and I was OK. I think this was because of my taking ibuprofen on an empty stomach. If the Zantac didn't help, which was seldom, I took Prilosec for 3 days and it was gone. Now, after the spasms, I began burping and having hiccups frequently, which was new to me, so I started on Prilosec 20mg once a day. I also changed my diet to conform with the diet for GERD and I think I'm beginning to get better, but it's taken a lot out of me because I've been on the same diet for a long time and all of a sudden I'm missing crucial nutrients that I ordinarily get. By the way, I"m 69 and have been healthy and disease free for at least 20 years. I want to discontinue the Prilosec because I've only been taking it for a little more than two weeks. I'm wondering if I could switch back to the Zantac 150mg twice a day without experiencing rebound symptoms because I've been on the Prilosec for such a short time. I can see that there are many people here with digestive health problems and I'm hoping someone can steer me in the right direction. My doctor is very good at giving labs and writing prescriptions, but he doesn't seem to do much doctoring, but that was OK, because I haven't had any serious health issues. He just told me " You know you have to see a gastroenterologist". Because of the holiday I haven't been able to schedule an appt. yet and now, with some of the symptoms lessening, I'm hoping there's a chance that I can switch to Zantac, or preferably cimetidine (Tagament) which helped me greatly when I developed an ulcer many years ago, and solve this problem without any intervention. The reason I want to stop taking it is because of the side effects. I've had diarrhea a few times and lots of headaches.I"m also worried about the things that can happen when you have so little stomach acid. I"d appreciate any comments from people who have had experience weaning off Prilosec after short term use, or anyone who has suffered with LPR or laryngopharangeal reflux, or anyone at all. I appear to have the symptoms of LPR - throat mucus and burning, hoarseness and sometimes feeling an object in the throat - and I very seldom have heartburn. This fits the description of "silent reflux" except for one thing. It isn't so silent. I may not get heart burn but I readily get bad stomach aches to accompany the other symptoms, especially if I ever more than a small amount. I have never seen this symptom mentioned in any LPR discussion. They just usually mention the absence of hrartburn

So my question is whether bad stomach aches is common in LPR or is something else possibly going on? (FYI, I've been scoped at both ends and everything looks normal. Medications don't help, PPI's, stomach motility, antacids, Gavascon, etc.)

Hi,
I’m hoping you found some relief. I also have LPR that is not silent. My main problems are sour taste in my mouth, sore throat, burning in my ears, soreness in my chest ( not sure if it’s my esophagus burning or a burn in my lungs), and labored breathing. I’m on 40 mg of nexium 2x a day and sulcrafate but I still get stomach aches which feels like acid churning.
Has anything helped for you?

@michelle34 That is a lot of Nexium. .. Have you been to a really good clinic like Mayo MN.. I saw Dr. Halland, an esophagus specialist 6 years ago .. He gave me 3 tests in the morning, and a way to cope in the afternoon.. I have had telephone consultations with him during the pandemic.. I have Gastroparesis as a result of a botched Nissen surgery done in Iowa to reduce GERD... Ken

Hi Ken, I’m sorry to hear about your surgery complications. I hope you are able to overcome this. Do you have LPR as a result of your GERD? If so what medications are you on to treat it? I was only able to have a telehealth appointment with a gastroenterologist from Mayo Clinic. He said it’s difficult to get a good evaluation over the phone. I’m on the east coast and I’m trying to figure out how long I would need to go there to get the proper tests and hopefully treatment.

@michelle34 Have you tried Boston General or the Cleveland Clinic..? Both are highly rated in GI.. Right now I am not taking any other meds for the esophagus ... things have settled down during the Pandemic.. as I am not on the go so much.. K

Hi, I’ve been suffering with LPR for 5 months. I’ve tried multiple PPI’s, H2 blockers, and antacids. I’ve changed my diet and elevated my bed. I’ve been to multiple gastrointestinal doctors and ENT’s that can’t seem to help me. I’m not getting any relief and my symptoms are getting worse. On a daily basis I have burning in my chest and ears, sore throat, constant sour taste in my mouth, post nasal drip, and asthma. I think the worst part is my mental health. I’ve been to multiple gastrointestinal doctors and ENT’s that can’t seem to help me. I’m not getting any relief and my symptoms are getting worse. @michelle34 I sent another reply earlier about Dr. Halland at Mayo MN (so look for that too) . He prescribed two medications, a 20mg Esomeprazole Magnesium and a 15 mg Mirtazapine (as he says, it will help you think you should eat) ..
