Does anyone use inhalation Tobramycin (Tobi)

Posted by lindabreathless @lindabreathless, Feb 12 10:37am

I have BE with pseudomonas and inhale, via a nebulizer, Tobramycin (Tobi) 2x daily for 28 days on then I’m off for 28 days. During the time on Tobi I am short of breath. Once I stop after 28 days, I feel better again and get my breath back. The inhalation does control the pseudomonas and keeps me from getting infections. Does anyone else experience shortness of breath from using Tobi?

Thank, Irene. I didn’t know that Cayston is now covered by Medicare. I was turned down a couple of years ago when my doctor prescribed it. Do you take it? If so, is it helping?

I hope to try it again soon. I tried it in the past and my heart rate went up quite a bit and I only ended up doing 5 doses (long story). I get fast heart rate easily. I just finished 11 days of Meropenem IV and when I asked my Pulmo about Cayston he said "not now". I want to try it again and need to get clarification from him as to why he said that.

Sorry to hear about your heart rate. It will be interesting to know why your doctor said “not now” to Cayston.

It was at the end of a tele appt and he had to run so it turns out he meant not now to discussing it. Spoke with him again and he said I can try it. Tried one dose yesterday. Developed chest discomfort for a long time after. So really not sure. Might adjust to it if I keep trying it but just not sure.

On Tobi now for 14 days 2x per day. I love it. I breathe great even after 1 dose.

Never heard of using it as a one time 10 day treatment. Why not test sputum afterwards at some point? People who tolerate Toby are so fortunate because it is very effective for Pseudo.

They can test your sputum to see if the pseudomonas is gone. Mine took 2 rounds of other antibiotics along with two rounds of Tobramycin to clear (in 2018) and hasn't been back.

This is my 3rd time for Pseudomonas. I am also on 300 mg daily but for 14 days. Then we retest after a week or so and either I am done or I re-start. It has always cleared it after one round of 14 days before. Just got over some weird virus that turned into Pneumonia. The Pneumonia triggered the Pseudomonas. Been down flat on my back for the last 7 days but sick for 24 days. When this is over, we start re-testing for MAC (had one positive culture and a negative something , I forget) and then there is the old Stenotrophomas Maltrophilia that we still need to determine if it is really gone or not. Been treated 3 times for this over the last 7 months. The last round of meds lost my hearing for about 3 weeks. It did come back once off the meds. The hearing aid guy said it is as good as it was before the meds. I am very tired and trying hard to regain my energy. It is coming along though but very, very slowly.

I also have used toby twice a day for 28 days, then 28 days off. It does control my pseudomonis. I get a sore throat and cough a lot, with some shortness of breath, all getting progressively worse during the 28 days. It all resolves about a week after I stop. Right now I have permission to see how long I can go between flareups without the toby instead of coughing every other month. So far so good, but I've only been off for 2 months.
