Diagnosis based on age 84 and PSA rise to 40

A friend was sent to a urologist due to a rise in PSA to 40 and age of 84. Urologist said he had prostate cancer. This was not based on a biopsy or other tests. I think the approach was considered "watchful waiting." I know there are treatments and that age is probably a major factor. I think he should request a biopsy to verify this. Comments and advice please?

At age 84 it would be a difficult decision for me to have a biopsy. After the biopsy confirms cancer then the next decision is whether to have treatment or not. Then if you have radiation treatment you have side effects that will diminish your life style and then begin to wonder if it would have been easier to go from the cancer or the side effects. My physician told me at age 79 there is no free lunch when you are at this age, surgery and radiation have side effects. Good luck in the decision.

With a number that high the diagnosis is probably correct. He will be lucky if the disease is contained in the prostate. The time for waiting is over, it's time for action.

@stoney @wrig1jam I was 81 when prostate cancer was diagnosed in me using the elevated PSA test, several biopsies over the years, and another new test that could only be read at Mayo or the University of Iowa Hospital... That final diagnosis was made at Mayo MN.. I had the 20 session radiation Oncology plus 1 Lupon (hormone) shot 2 months prior to the radiation to shrink the prostate.. That was nearly 2 years ago and my PSA now is 0.5 for that time since checked every 3 to 6 months.. The Oncologist and the Urologist think that I am doing well... I certainly have a few side effects but life is good.. Ken
