Chronic pain in Neck, shoulder and arm

My wife has this chronic pain that was faced since 2021 to her neck, RT shoulder and RT arm. She is 31 years old now and still having the same pain and we can say that gets worse with time.
Her symptoms started on Dec. 2021, with stiffness and pain on her RT hand especially on her thumb, after a while swelling started to appear on her hand and a circle-like bump sometimes appears which makes the pain level to spike. One year later and during that year, the pain expanded to the rest of her fingers, elevated to the elbow and shoulder with severe pain, weakness, numbness, swollen shoulder blade, and her arms weren’t raised more than 40 degrees. After visiting 42 different doctors specialized of different fields, all tests and MRIs were done, she was diagnosed with a mild compression on c5-c6 nerve root, thus she had an ACDF to her spine on Jan. 2021.

Three months after the operation, and after attending 40 sessions of physio therapy, the same diagnosis appeared but without the numbness symptoms. Doctors had their doubts that she might have a frozen shoulder, thus she was injected with cortisone shots twice on the RT shoulder and once on the LT shoulder and she wasn’t getting better at all. On Jan 2024, she performed a capsular release surgery and no frozen shoulder was diagnosed while operating the surgery.

Unfortunately, all inflammation that were removed during the last operation, got back worst than before. All doctors said that don’t know how to cure her.

Blood tests have been done, rheumatoid factor and ANA test; all numbers were normal within the average.

Till now, her symptoms are getting worse than ever.

Hello friends, I have just signed up here, and please forgive me if this is not the correct spot to add my story, but I am desperate for help.
My right shoulder and down same arm and neck have been hurting since November, and this was not following any trauma. It exacerbates after activity, and sleeping , if I lay on that side too long.
I am right hand dominate, I use a tablet for home health job, and type on keyboard frequently. I had a huge flare up in Dec (25) . After a cruise, when I felt it exacerbated from dragging a suitcase onboarding, I went to a walk in ortho clinic upon return Dec 16, They x rayed the arm and shoulder multiple views ,and was surprised to find this is NOT my joints as in bone on bone . The P.A. said he thinks it is a muscle sprain , and gave me a Prednisone pack, He also asked would I like to go to P.T. ( Physical Therapy) , I said yes, I got thru holidays first, and then we got major ice ,snow in January, now been to P T sessions x3. The therapist disagrees with the P.A. and thinks it is rotator cuff. My Range of motion is good, and so we have been doing these little exercises now for 3 weeks, and today I finally see a Orthopeadic surgeon . I hope for some answers and some real relief, maybe a cortisone shot , anyone else have any ideas , and I also have neuropathy in same arm and hands, Numbness and tingling, I am not diabetic. I am desperate for answers though , and I will report back tomorrow to tell you what the DR finds. Thanks for any ideas and thanks for listening.

Hi- I had something similar that started 2 months ago, was always worse in the morning. Deep massage on my trap and neck helped temporarily, but was not lasting. I finally discovered that if I slept with a heating pad on my shoulder and neck it was not as bad in the morning, and I would massage it throughout the day. I saw a chiropractor who did massage with active release which also helped a lot. i sleep with a very small soft down pillow that I mainly use to support my neck which also has helped. Sleeping on my back is best. The best i can figure out, this started due to a bad shoulder (rotator curl tears) that kept me from sleeping on my left side, so I was instead only sleeping on my right (my pain is on the left side)

@heartnurseva So sorry you’re in pain. Ask for an MRI if it doesn’t improve. There are a lot of issues that can cause this pain trajectory that cannot be seen on an x-ray.

I find acupuncture helps and Botox in my head - cause is multiple injuries over the last 60 years. Also in physical therapy to improve neck mobility- rather amazed that I regaining some flexibility. Refuse surgeries.

@kdks99 Thank you so much, I have had the MRI a week ago, just waiting to hear if the ortho surgeon wants to operate, It is a rotator cuff tear

sounds horrible (as I sit here in the same kind of pain!) Try botox in the muscles which are impinging on the nerves causing pain, try MRF (that is SUPER helpful for pain relief - kiss a lot of frogs to find the prince!) and try low dose opioids (I like percocet) if she doesn't have an addiction issue. Acupuncture. I am in P.T. now and it only seems to exacerbate my pain and muscle spasms. WET HEAT and stretching. Try sleeping on a rolled towel. I did that for maybe 6 months straight and it TOTALLY helped! I am going to start doing it again tonight. I had two cervical surgeries - on both sides. So sad it didn't resolve my problem. Oh - and wear a soft $10 whiplash cushy brace when she gets tired (when the muscles get tired) that is GREAT. I was good for about 30 years and then I wasn't. Seems my auto accidents and whiplash added up. It can take a couple of years to heal. My last whip lash was 6 weeks ago. Good luck.

@susanblan Myofascial release (MFR) is a specialized, hands-on physical therapy technique that uses gentle, sustained pressure to release restrictions in the fascia—the connective tissue enveloping muscles, nerves, and bones. @loriesco yes, I have doing this for past month. Early each mornng then later in afternoon. I suffer from ON my neck, muscles, at tight. I feel better!!!

I suffered for years. All doctors had no answers and one told me to seek help because there was nothing wrong with me. But I did not give up! Found a great neurologist and he suggested genetic testing. And he was right! I have Hereditary Nerve Pressure Palsy. A very rare condition. It also falls under Charcot Marie Tooth CMT. They suffer the same symptoms. I would definitely ask for genetic testing. He told me I had either Lupus, MS or HNPP and he was correct. Not much I can do for it but be in the care of pain management doctor. Shots, nerve blocks and ablations have helped. Hope this information helps. And yes I did go for the MRI for brachial plexus which was normal. Don’t give up. Insist on genetic testing.

@terlato Good for you. I suffered with my ON for a very long time, before I found the right combo to help; Nerve shots, triggers shorts, clinical massage therapy and learning that I can use exercise (mfr) twice a day, (Dr. Jon Saunders) and others on u-tube I learned more than going to PT. But I must do it every single day. The Cervical log Pillow was my #1 helper. Feeling positive
I know I cannot cure my issue but I can certainly assist myself. I am so sorry you went for some time not know. The very Best to you, from a sufferer.
