Breakthrough bleeding, but been on birth control for 2 yrs (pls help)

hello. i am a 19 year old female. three or four days ago i noticed i was bleeding even though i had seven days of pills left before the placebo pills. it’s not spotting either, it’s a full on period. like i am wearing tampons and when i take them out it doesn’t feel dry or anything. is this something to be concerned about? i tried looking it up online, but all the stuff on breakthrough bleeding only applied to breakthrough bleeding during first few months of being on the pill. i have been on this birth control (ocella, offbrand version of Yazmin) for two years. i didn’t skip the placebo week or anything. i don’t take it at the same time everyday, but i just take it when i wake up and that has never caused any issues for me. some days i miss taking the pill (not often) but again that has never caused problems for me. i read somewhere it could be a miscarriage? i let my ex come in me like two weeks ago (we were dating at the time). i have let him come in me twice before and take a plan B afterwards, but this time i did not take one because I read you should not take plan B if you are on birth control. could it be a miscarriage? or am i totally overreacting. please let me know if this is something i should be concerned about.
