Anyone dealing with gout in the hands?

My husband has been diagnosed with gout in his hands. He is in PT and has seen a hand Dr. OT and surgery has been suggested. He hasn't had a gout attack, surprisingly. He has had it for about 4 years but is has gotten worse. Has anyone in this community had to deal with gout? If so, I would love to hear your experience and suggestions.

My mother has it bad, but mainly in her feet. She takes daily meds for it. A family friend used to have it in her fingers. She did have surgery for it, but I don’t recall the details. Sorry. I know it’s very painful from what I have seen in others.

I had a first gout attack over twenty years ago, in the feet, didn't even know what it was for a long time, didn't even mention it to a doctor until years later.
I had a couple of other gout attacks over the years until I got it largely under control by diet and supplements, but at some point I guess one knuckle on my right hand swelled up, I don't think there was ever any pain there, but it's a "tophus" from gout, a buildup above the joint. It now seems to be shrinking, very slowly.
My uric acid still runs a bit high.
