Anterior Ethmoid Neuralgia/Nasocilliary Neuralgia

Posted by jtr1 @jtr1, Jan 18 12:09pm

I have had chronic pain across the bridge of my nose for the past 9.5 years following a septoplasty and sinus surgery. The pain is 24/7 and a 9-10/10. It very much mimics the symptoms of anterior ethmoid neuralgia and that is the area the surgery took place. There plenty of articles on this condition but I have been unable to find a doctor who treats it. I live in Texas currently and the pain never goes away. V2 and V3 involvement have been ruled out. Does anyone have a recommendation of anyone who treats this condition in the states? The pain never goes away

@jtr1-

That is a long time to be dealing with that without any resolve.

Are you affected by fluctuations in air temperatures. Hot/Cold air?

Is there anything that provides somewhat relief?

What has your CT scans of the sinuses shown?

When you inhale is it always clear?

Have you seen a Neurologist?

Nothing changes the pain. Hot or cold does not really impact anything. CT just shows what previously operated on sinuses look like. It is clear. I have seen a neurologist and have not found relief through any medication or other remedies. My symptoms match nasocilliary neuralgia.

Do you feel any of this in your teeth?

Do you feel any type of bend in the very anterior of your septum?

Did you have any additional nasal work?
Inferior or Middle Turbinate work?

No, it is completely concentrated on the bridge of my nose. The pain never stops is very intense. During the surgery all of my sinuses were operated on including my sphenoids and ethmoids. It is not related to the SPG nerve either as that block does not work along with many other v2 block I have had. I can’t feel a bend in my septum.

Yes, all of my turbinates were operated on. I had a septoplasty, complete sinus surgery (including haller cells), and a turbindectomy 9 years ago. I have had debilitating pain across the bridge of my nose ever since. I did not have pain before the surgery.

That’s pretty extensive.
I only had one middle turbinate altered w/ septoplasty and partial sinus surgery. I have nasal bridge pain constant. But I also still have sinus inflammation. However the pain I experience is disproportionate to sinusitis as I never had this pain before.

Does the pain ever make you nauseous like migraine type?

I do not get dizzy. Just horrible pain. Distribution is attached.

@jtr1

With the pain do you still have any nasal sinus symptoms like post nasal drip, dryness etc?

Have all of your sinus issues resolved and you are just left with pain?

I wonder if the pain is lack of any positive pressure and mucosal contact points?

When you pinch your nose on this mid vault bridge area does the pressure applied alleviate the pain while it’s pressed on?

REPLY

I do not have any sinus symptoms at all. It really does match anterior ethmoid neuropathy/ nasocillary neuralgia to the description. All I have is the pain. The bridge of my nose/midvault area is very sensitive… I can’t wear sunglasses since I’ve had the surgery. That’s the general area of the pain
