HRT and abnormal vaginal bleeding

I had some hormone therapy last year (implanted estrogen) and failed to take the progesterone pills because I couldn’t swallow them. I should have asked for a cream but didn’t and now I am having bleeding, not profuse, but enough to be worrisome. I. Am 81 years old and never had any gynecological problems. I used a vaginal cream of E-3 .05/TE .25 for 10 years with no problem. But my primary doctor decided I couldn’t have that anymore so I went to another clinic. This was all bio-identical hormone therapy and I believed it was totally safe. Now this. The bleeding has been going on for several months and I have an appointment the end of April but I’m starting to worry that maybe I should go in sooner if I can get in. If I have cancer I’m going to really blame myself for not using progesterone with the implanted estrogen. I don’t really have any friends or family to talk to about this so here I am. Should I try to go sooner than end of the month?

Hi Jen,
I would go in earlier. My doctor told me that if I had any bleeding at all, to call him right away. Especially since you said the bleeding has been going on for several months. I'm not in the medical profession at all, and I don't know a lot, but I do know my doctor said to call immediately. I hope you're feeling okay, though, and I hope you get seen soon.

Hello.
I started HRT with progesterone a little over a year ago.. a year into the treatment I started having abnormal bleeding. They did an ultrasound and found nothing. The bleeding continued for 6 months. A second ultrasound showed polyps and thickening of the uterus. I went in for a hysteroscopy and D&C which was a simple procedure don’t under local anesthesia. I had mild cramps and spotting after. The biopsies showed grade 1 B1 uterine carcinoma. I will have a full hysterectomy in May. Thankfully it’s slow growing and the most common type. I encourage you to advocate for yourself and see a doctor. My oncologist told me if her patient is older often times after the diagnosis they do nothing because of how slow it grows. I know it’s scary not knowing. I was terrified and still have the surgery to go through. I hope you get some answers. Will be thinking of you. 🤗

try to get in sooner. i had abnormal bleeding but it took a month to get an appointment at my gyno and by the time i saw her i had lost so much blood that i was severely anemic and had to be admitted to the hospital for a blood transfusion.

@jen81 I very much agree with @suesam, @klue, and @sbryant. If I were in your place or you were a relative of mine I'd advise calling the gynecologist's office and asking for an earlier appointment. You can also ask to be placed on a waiting list if someone cancels. The abnormal bleeding could be related to the hormone therapy you had or it may not be. At this point in time any bleeding when the woman is postmenopausal should be carefully evaluated.

To be honest, I'd probably go to the ER too. They have all the equipment there to see what's going on. Wishing you all the best and hope you're doing okay.

My ‘vaginal’ discharge/bleeding began June 2024. My gyne told me it was from old age (70) and dryness and friable tissue. Gave me estrogen cream. I had a total hysterectomy in ‘95. The bleeding continued and in January 2025 had biopsies as now she saw a ‘lump’ down there. Came back stage 1A squamous cell cancer due to HPV which was the cause of my hysterectomy. Will be getting radiation/chemo.
Any woman who is over 70 and has had a hysterectomy with a complaint of bleeding needs to be assessed immediately and taken seriously.

@leahchawkins Thank you for this. I have had a hysterectomy due to endometrial cancer. If I develop vaginal bleeding I will insist that it be investigated with a complete physical exam.

Yes… we must become our own advocates. I waited and waited for a referral finally to a local gynecologist/oncologist (we live 6 hours away from Mayo) and after 4 weeks of no call, I called Mayo, had an appointment within 4 days and surgery under GA 5 days later … all the best to you 💕💕
