3rd knee replacement in a year and a half on same knee
I had a total knee replacement in May 2023, with pain and swelling and was unstable. Surgeon said I had to have a revised knee surgery 6 months after the first in. He told me that he didn't put the cement in the bone for the implant to be stable. After the second replacement I continue to have pain and swelling he told me that all x-rays looked good. I have been to 6 different rehabs for therapy with no results. 4 weeks ago He said that I needed to have the spacer replaced because he put the smallest one in and it needed to be higher. I am still having pain and swelling. Now I don't know what to do. I am a 65 year old female that was active be for the first knee surgery, now it hurts to stand from a setting position and feels like a tight band around my knee. If you have any information that will help I will greatly appreciate it.

Be sure and have a complete blood metal panel run before deciding on a replacement implant. Test also for bone cement allergy. Medicare won't pay for this $600.00 test, but it's the best $600 I ever spent.
can you please tell me how your knee felt before and after the revision to scrape scar tissue? can you also tell me how the procedure went and how long you were out of commission? thank you so much for any help, I had TKR Oct 2023 and still have pain swelling tightness and they are recommending the revision to scrape scar tissue but I am nervous about doing that
I had my TKR done in Rochester, NY.
I ended up with excessive scarring (Arthrofibrosis). They then did an MUA, followed by Arthroscopic Debridement (for scar tissue removal) and finally an Open Debridement. After each procedure, I ended up a bit worse, with limited ROM, pain and inflammation. Having checked and losing faith that a doctor in Rochester had the necessary experience to correct my condition, I made an appointment with a doc at The Hospital For Special Surgery (HHS) in NYC. There, you choose your doctor online ahead of an appointment, which gives you the opportunity to review their areas of expertise, education, experience, review articles they have written and see reviews. At HSS (#1 worldwide in Orthopedics)they advised revision to a “Hinged Knee”. Since HHS does not accept insurance, I then chose a doctor online at The Mayo Clinic (#2 Worldwide in Ortho.) for treatment. They declined to see me, citing that they have many more people trying to get in than they can handle. I then searched doctors at NYU Langone Orthopedic Center in NYC (#3 Nationally in Ortho. I visited there, where Dr.Bosco recommended revision to a Constrained Condular Knee (CCK) . With severe Arthrofibrosis, the more common approaches are either a hinged knee or a CCK. I am now scheduled for the surgery in May. Dr. Bosco feels that the surgery will increase my ROM significantly. And Langone takes Medicaid and many other insurances. I hope this helps. Rochester, NY is not a small town and I still was unable to find anyone here experience in these more complex procedures, so I feel for someone coming from a small town and in need of expert treatment for something uncommon, as severe Arthrofibrosis affects under 5% of those having TRK’s. That said, it was important enough for me to bite the bullet and travel, which I found necessary. BEST OF LUCK TO YOU!!
Revision or second total knee replacement. It is the same amount of time as with the first one. Before you do anything please, please get a second opinion before you go back to the same doctor.
The revision takes long in the operating room. Because they will put in a larger one, have you seen your x-rays, get copy's to take with for a second opinion and all medical records. I am still in pain, I can sleep at night The pain go down to my foot. God be with you. I hope this helps.
Thank you for your comments, now I am just getting angry, you would think they would know what needs to be done.
Other doctors don't like to work on a previous surgeon work, they say it all look great, then why are we still in pain? They can not answer our question. So you are left wondering needs to be done next. Sorry it just upset me they just say well you need another surgery. Not right at all.
I had a knee replacement few years ago was always tight had Mua at 6 weeks …. Did not help 1 year after my tkr had revision with another dr he went two sizes smaller felt was too big … been two years since revision my range of motion has gone back 65-70 flexion this is where I we ended up after original tkr …. Went to my dr he told me this never should have happened with going two sizes down …..surrounding tissue no matter what I do will always take me back there …. I trust him that is why I am done with surgery..,now I try to find what gives me comfort …. , I have red light therapy at home has been helping tremendously with pain …,chiropractic helps with cupping and laser therapy I still exercise …. I have not given up … I just refuse to get into that cycle because scar tissue does not get better for me it gets worse every time …. So yes I am cutting my loses here at two surgeries and a mua…. As far as my other knee go I am doing more conservative approach…..I have bone on bone and I am 62 and very busy with grandchildren…. But we all have to do what is right for us …..good luck to everyone that is still searching hope you find that right Dr. that can help ….
Go ahead with the scraping. I could not bend my knee at all And the doctor said he didn't think I would get more than a seventy degree bend. I told him I couldn't live with a straight leg so had to give it a try. I now have 110 degree bend. Dr was amazed at my progress. Recovery was a bit easier than with the total knee replacement. The doctor put in a dozen large suitors to hold everything together in my knee. Those sutures caused me a great deal of pain. A year later he removed the sutures and now things are much better. You will need physical therapy all over again. When I was able to swim again doing exercises in the water really helped. I also use the hot tub at my fitness center which makes a big difference too. I also use fins and a kickboard to do laps which really works your quads and your knees. that has made a huge difference. Good luck.
I had my appointment with the sergeant yesterday. After looking at all the results and x-rays of my back he came in and told me that my back was in bad shape but he didn't think it had anything to do with my knee. He got a report from the doctor that did my second opinion stating that the tendon that runs from the side outer side of the knee down and around the front is what's giving me the problem that it's irritated and swollen and he wanted to give me a cortisone shot, I told him that he had given me a cortisone shot the first time I was having problems with the knee and it did not do anything. And I asked him if there was any tests that can be done to find out for sure without guessing and doing certain things that probably would not work. He then told me that he could do 1 of 3 things a cortisone shot, or he can go in and debrief the knee or he could go in and release the tendon and clean out the scar tissue. I told him I didn't want to do anything until he knew for sure if this was in fact the problem so now he's getting me scheduled for an MRI so that he could see the tendons and the irritation around the knee itself. So now just waiting to hear back to have the MRI done.
Thank you for all of the above information. When I had my TKR 10 years ago, it was great. ROM was always good after therapy and it was all fine until the prothesis itself is breaking apart in my knee. This started a year ago and it has still been ok for the past year but now it is getting worse and more unstable and doesn't pinch like when you had no cartilage but it feels tight and unstable. I still walk around but don't know if that is making it worse or I should just continue like I am until it gets worse.
I was in a lot of pain before. After not as painful. Was able to pt and steadily improved. I personally rented the flexionator to bend my knee which really helped. Also bought a recumbent bike which helped. You should definitely do the surgery. I had nothing to lose. Knee still stuff but can bend 110 degrees and walk without a limp. I can go up and down stairs slowly. I was a very active person before all this started so disappointing that I can't run, play tennis, hike. But I don't have the constant pain and can function.
