Anterior Ethmoid Neuralgia/Nasocilliary Neuralgia

I have had chronic pain across the bridge of my nose for the past 9.5 years following a septoplasty and sinus surgery. The pain is 24/7 and a 9-10/10. It very much mimics the symptoms of anterior ethmoid neuralgia and that is the area the surgery took place. There plenty of articles on this condition but I have been unable to find a doctor who treats it. I live in Texas currently and the pain never goes away. V2 and V3 involvement have been ruled out. Does anyone have a recommendation of anyone who treats this condition in the states? The pain never goes away

I unfortunately have nasal/sinus symptoms still, however I keep telling them that if this is what inflammation/infection feels like, then I never had inflammation/infection pre surgery.

They say I could now have sinus issues that have caused overly active nerves.
But they can tell what is what due to the way my sinuses look.

I just know, that never in my life did I have pain like this.

Do you get any pulsation pain that ever goes to the back of your head like migraine or is it always just at the front?

Has your Neurologist treated you as a “migraine type” patient?
Have you tried amitriptyline?

I do not have any migraine symptoms at all. Nothing to the back of my head. Literally just very intense nasal pain 24/7 from a surgery

Any intranasal lidocaine help in that area?

I haven’t had a doctor hit the spot yet. I’ve been working on finding a doctor who treats this condition, can do a test block, and then either ablate or free the nerve

Yes, for this condition you need it injected to hit the nerve a topical spray does not help

I know this has to be frustrating for you. And I’m sure you’ve tried everything.

I don’t know who specifically can do this. But there are some ENTs that are experienced enough and understand how to do certain things and sometimes work along side their neurology team to help with the nerve issues.

Facial pain post sinus surgery is so challenging to navigate as everyone wants to blame it on inflammation/infection.

Have you had any exploratory surgery under GA?
