Questions about MRI report

Posted by scottrb43 @scottrb43, May 9, 2023

I had an abnormal PSA for my age and so I was referred to an urolgist who did a digital rectal exam and didn't feel anything so I had an MRI done and it says there is a subtle T2 hypointense lesion of the left posterior prostate gland measuring 11mm it is associated with slightly increased DWI signal and decreased ADC signal. Enlargement of the prostate gland measuring 5 by 4.5 by 3.6 cm They can't get me in for a biposy fo several weeks. SHould I be worried that it's cancer? I not only have incontinence and impotence issues but I have had a dull aching pain in my upper thighs and what feels like a blood clot in my calf. Any advice would be greatly appreciated, Thanks!

The incontinence and elevated PSA may be due to BPH. You can try anti inflammatory drugs to see if the problem is reduced. But at the end of the day, you can have enlarged prostate and cancer. Biopsy may not identify the cancer. PSMA PET scan is more certain. That plus MRI is 98%.

A research team in Australia recently conducted a test to see if the PSMA was the gold standard for detecting PC. Their findings published in the 2022 EAU annual congress,(Amsterdam, The Netherlands) They compared PSMA to mpMRI, 240 men were tested. The MRI had a 75% accuracy rate and the PSMA 62%. Lead author DR.Lih-Ming Wong, observed,"our analysis found that MRI scan were better than PSMA PET for detecting any grade of PC. The difference is considered statistically significant.
