Benign prostatic hyperplasia (BPH) Surgery Options

I had a Urolift procedure last February for my BPH symptoms and it was a total failure. The implants are still in position though. I am looking for a different procedure to alleviate my BPH problem and Rezum seems to be a good alternative. Can it be done considering my prior Urolift procedure. If not, what would be your recommendation.

I think this is a good place to jump in as I was planning to report my latest decision. I had been hoping to do an Aquablation at the Lahey Clinic here in MA, doc has a lot of experience. But since I had a 125cc size a few years ago he sent me for a CT scan to get a more recent accurate measurement. He doesn’t think Aquablation should be done over 160cc. Well, I hit the jackpot at 229cc, lucky me. I do manage it well with Tamsulosin but it’s time. He recommended HoLep or PAE, the latter was my consideration for awhile as I am not willing to risk the 2% incontinence factor with HoLep. I am going for an evaluation this Friday. BTW, the record for largest prostate goes to a guy in 1918 who came in at 706cc. No Guinness Book mention for me.

I had a PAE a few years ago and the results were superb with ZERO side effects. Unfortunately the prostate continued to grow and about 3.5 years later I was back on meds. But the meds now work better than the did prior to the procedure. I'm told too that sometimes the PAE results are permanent and solve the problem for good. Just be sure to get an experienced doctor as you don't want to impact other organs by accident. The procedure was painless and I watched on the monitor the entire time. If the prostate becomes unresponsive to the meds again, I will consider another PAE.

We have United Healthcare Medicare Advantage and Rochester Mayo accepts ours. We just pay a copay.
I am supprised that not all locations are the same.

I live in Florida, I received a letter from the Mayo stating that they would not accept Advantage Plans going forward. I have Medicare and an AARP United Healthcare Supplemental Plan…..which would be accepted by Mayo.

All I know is we were sent a letter that stated as of January 1 they would not accept UHC Medicare Advantage. However, since they are still monitoring my PSA after radiation they haven’t kicked me out yet and accept the insurance. But I’m not sure about other departments.

I may be missing something already mentioned here, but I think Mayo will NOT accept
UHC Medicare Advantage Plans
but WILL accept
AARP United Healthcare Supplemental Plans.

It might just be me, but this is one more example of how things are changing too much, too fast for my aging brain.

I had the HOLEP Procedure in May do 2022. If you have any questions about it I’ll be happy to answer if I can. I am a proponent of this procedure, could not be happier with the result. I’m glad you’re doing your research!

@kayak461 Thank you for the reply. I read you blog from last May after your surgery. Your experience gives me hope that I can leave this behind and enjoy life without planning my day around where are the bathrooms located.
I assume you are still doing well after almost a year post surgery?

@bill5letsfixthis This an excellent video that answers many questions about HoLEP. It sounds like a better way to go than Rezum. My Rezum was done about 5 months ago so is probably as good as it's going to get. The flow rate is unexciting but OK. I still get up at night a couple times. Flow rate at the end of the session is low and takes time to empty. I have left Myrbetriq in favor of Oxybutinin. The Oxy gives me about an hour and is affordable.
