Finding good Dr., medication discussion

I'm 74, have had 2 Traumatic Head Injuries (on the front right side of my head, 1966 & 1996) I was diagnosed as bi-polar around 1997. I've had terrible experiences with the Psychiatrists I've worked with over the years. The last just decided to quit and retire, with absolutely no recommendations who to turn to. Wouldn't even return my calls! I was with him for at least 18 years. And, as I understand, he left NO notes about my care and meds. So what I need from any of you is a recommendation or advice about finding a good & reputable Dr. (in the Littleton Colorado, 80123 area) Is this possible? My memory is starting to become bad, and I am responsible for my handicapped (borderline retarted, can't read or write past the 3rd grad level) and I'm scared. I am presently taking 100mg Lamictal and my Primary Care started me on 7.5 mg Mirpazapine on 1/24 for sleep and depression. Hasn't helped yet. Getting terrible headaches after taking. Any thoughts about this combo? I truly am afraid and need to find a good Dr. Any thoughts? Thanks so very much.

So legally he does not have to provide his clinical notes UNLESS requested by a judge but often they will share and consult with a new psychiatrist. Have you tried having your PCM requesr all records for coordination of care and complete medical record?
I have worked behavioral 11 years as a facility manager. I am certified in medical adminustration, medical transcription and master billing and coding.
If you saw him for so long he may not want you to see his notes in fear of triggers or re exposure depending on your situation. You always havr the option of requesting your complete medical record through the governnent but again he is only required to provide notes to: a judge and: your insurance for billing should they request .

Now as far as meds: usually takes 2 weeks to acclimate but if your headaches are too bad id recommend seeking evaluation services for different meds because headaches can be way too many things. Honestly it depends on your insurance but I would just find a psychiatrist that will order your records as coordination of care, change the meds.

So TBI qualifies you for case management also and your case worker can nicely request records and honestly: case management for neuro or trauma anything will make your world SO much better. Have you done EMDR or exposure therapy? Have you been to a chiropractor? Headaches and bipolar can trigger many other symptoms simply because of their symptoms. I would search for a nice TBI inpatient and let them fully evaluate, treat my tbi bipolar symptoms and observe me and give me care. Often those centers are much more focused on the care you need and i would have an open discussion and request a clinicuan willing to fully disclose but just know: they dont have to.

Also : lack of sleep, tbi , headaches , racing heart and recurrent thoughts : Have you considered a functional neurologist?! I ask because in 2 sessions this girl had me retraining my eyes with dots and i felt amazing for a very long time it countered my head symptoms entirely.

I dont know your income or medicare coverage but this is a waiver that will allow you to receive various services and treatments.

Cognitive issues and even learning issues caused by head injuries or a pre existing trauma of any kind can be treated . I worry that you are presenting pretty significant descriptions for your injuries being so long ago... have you considered a rehabilitation center for brain or neurological because they have programs designed to combat the trauma and thoroughly make sure you arent developing dementia etc as your risk is higher after an injury and mental health medications.

It may be worth applying for the waiver and having in home intensive for tbi and brain strengthening . Memory loss and learning retention or even coexisting is so common but you qualify for care based on your age, trauma, symptons, and diagnosis you may truly benefit so it doesnt progress quickly.....by presenting symptoms for so long : trauma can manifest and dementia risks are so much higher. Private organizations will even send you on intensive tbi retreats which are considered luxury with high yeild neurological, diagnostic, rehabilitative and patient centered care that you can truly see life changjng differences in: and most of them offer financial or charity plans and if they dont you qualify for case managenent and authorization via state funding on a case by case basis. you can get.

Last one: This center locates funding and has very high clinical care coordination AND patient reporting markers. I pulled on portal : because of your age, 2 tbis and the new act that passed and your symptoms worsening you qualify for case reviews , full disability and depending on your income just flat out 100% trauma informed compassion case reviews and treatment.

Sorry I am passionate about this as often this lack of provider coordination of care can manifest in a brain injury and nobody wants to feel worse: tbi itself qualifies as 100% disabled with symptoms that are clinically proven to advance faster if not treated. Another option is participating in studies. Tbi luxury retreats covered based on necessity. Your age , diagnosis , presenting symptoms qualify you as necessity. Please let me know if you want more resources because you even qualify for transport. Quality care is important with TBI and coexisting cognitive, mental health, and disabilies from a self occurred injury can be categorized as at risk anyway which really cab give you the care and access to GOOD doctors that care about your success and daily life. Ok im done 🥰

Thank you so much Colleen, I appreciate your quick response. Since I am a new member, I'm not sure you'll receive this. So I will sent this note again in several days. I live in Littleton Colorado 80123 and would appreciate any referrals you can provide. The real problem I have is a tremendous depression, making me unfunctional in many ways. I am with United Healthcare AARP Medicare Advantage Plan 1 (HMO-POS). I'm truly not sure how to access the benefits you cited. Not sure just what is covered by my insurance! I think it would be most helpful to have an Advocate to help me figure this out and to get the help I need. I would even consider exploring any option that would be available for me to come to the MAYO CLINIC for treatment. Yes, I am concerned about dementia with the head injuries as well as the use of Lamictal/and other previously used antidepressants.
