Recurrent prostate cancer: It's come back
In 2012 I had a positive Biopsy for prostate cancer. Initial Gleason score of 3+3. Chronology is as follows (w/Chart):
*Dec 2012 - Brachytherapy PSA score decreased to 1.0
*Completed 90-day PSA testing for 2 years.
*PSA lowered to 0.5. Physicians pushed my PSA testing to 6 months, then annually after year three (2015).
*In 2017 my PSA increased to 1.0.
*From 2017 through 2021 my PSA increased to 4.2.
*Feb 2020 Bone Scan, results negative.
*July 2020 PET, and Biopsy. Biopsy negative. PET imaging was inconclusive. Later discovered the Seminal Vessels were not biopsied in 2020.
*Oct 2021 additional PET and biopsy. Biopsy revealed cancer in one of the seminal vessels. PET scan was again inconclusive. Based on biopsy results, a salvage prostatectomy was scheduled.
*Dec 2021 PET Scan conducted with PSMA Trial AT CCA Seattle to locate any advancing cancer spread. PET scan was again inconclusive.
*Feb 2022 Salvage Prostatectomy
*Post-surgery pathology reports revealed no cancer in the seminal vessels or prostate. Meaning the Oct 2021 biopsy findings were a false positive.
*Post-surgery PSAs are 6.2, 4.9 & 6.1 respectively.
*Presently under 90 day PSA surveillance with a prognosis of hormonal therapy treatment upon PSA reaching level 10.
Anyone with similar experiences please share. Your information is no doubt invaluable.

Same get on Zolodex after Lasted years Today now going castrate resistive so adding Enzalutamide chemo in a pill
My Bone scan and CT scan show small growths in T10, T11 and left hip socket with 1" x 1 1/2" of metastasis in L1 lumbar My L2 was radiated almost 2 years ago and put back on Zolodex injections every 3 months which will continue
In Jan medical oncologist medicating me with 3 daily pills of Enzalutamide oral chemo for life and they may use radiation on L1 and other spinal metastases
The say good probability of 5 years and maybe more I'm 78 Prayers so important

3 ReactionsWhat a tough journey. Mine pales in comparison but I do share some similarities. Biopsy showed 4+3=7 w PSA at 80+. I went through EBRT
( ext. Rad not int Rad) for 2.5 months. May- July 8 2022.
I thought it wise to get a 2nd opinion at Mayo. Pathologist reexamined biopsy and reported a higher % of involvement and PSMA confirmed and included mets to pelvic lymph nodes. Went on Zytiga (Sept 2022) PSA is now 0.1. I have my next PSA test on Tues. My faith in Zytiga (after 4 mos) has me anticipating a PSA of 0.05 or less. Good Luck and Merry Christmas.
I think that with a PSA of 10 that your doctors should consider lupron and perhaps Erleada or some other ADT treatment. You might consider a second opinion as to your future treatment. I personally would want a more aggressive medical care.

Current plan is to start hormonal therapy when my PSA reaches 10. I assume this approach targets metastatic progression. I'm often confused between the interplay of metastatic versus advanced diagnosis. Thank you for the reference discussion.

Yes stage4 metastatic castrate resistant bone spine and hip
So my stage 4 castrate sensitive is going castrate resistive In Jan after our Aruba cruise medical oncologist putting me on Enzalutamide 4 daily chemo pills Prognoses if hopefully 5 years or more I am 78 now

My husband had prostrate cancer in 2003 at 62 and surgery, doctor told me to tell him to find something else to die from, amazing. 2011 he had radiation, so far PSA tests have stayed relatively low but have been slowly increasing still below 5. Whew! He sees a medical oncologist every 6 months who orders blood tests including PSA. He also gets a CT scan every 6 months as one of them showed he had lung cancer in 2018 but it was caught early and he had radiation. It’s a full time job keeping up with tests etc but one has to be proactive one cannot rely on the doctors alone any more.
